Abstract

Introduction: This study analyzes the neonatal outcomes of the appropriate for gestational age (AGA) twin from a diamniotic-dichorionic twin pregnancy in which the second twin had an intrauterine growth retardation (IUGR) and for which there was an indication to trigger prematurely the delivery. Materials and methods: To do this, three groups were compared: the first was composed of AGA twins (study groups), the second of AGA twins born from diamniotic-dichorionic twin pregnancies not containing delay IUGR (control group) and the third of small for gestational age (SGA) twins (IUGR group). This analysis was performed on eight items (respiratory, neurology, digestive, infectious, metabolic, hemodynamic, ophthalmological and growth) between January 1st 2006 and December 31st 2015. Results: A significant difference was found in the analysis of the respiratory help during the neonatal period between the study group and the IUGR (p = 0, 0116), but none among the other groups. The other items did not show any significant differences in their comparison except for metabolic disorders that are more common in the IUGR group. Conclusions: in conclusion, it appears from this study that AGA twins, for whom the birth was triggered prematurely following the indication of fetal distress of their SGA, have a similar evolution to AGA twins born from pregnancies not containing IUGR and having the same gestational age. On the other hand, the SGA subjects have a better respiratory outcome than the AGA subjects. Introduction : Cette étude analyse l’évolution néonatale du jumeau eutrophe issu d’une grossesse gémellaire bichoriale-biamniotique dont le jumeau souffrait d’un retard de croissance intra-utérin (RCIU) et pour lequel il y a eu indication de déclencher prématurément l’accouchement dans le cadre de ce retard de croissance. Matériel et méthode : Pour ce faire, trois groupes ont été comparés : le premier composé des jumeaux eutrophes (groupe étude), le deuxième de jumeaux eutrophes issus de grossesses bichoriales-biamniotiques ne contenant pas de retard de croissance intra-utérin (groupe contrôle) et le troisième des jumeaux hypotrophes (groupe RCIU). L’analyse a été réalisée sur 8 items (respiratoire, neurologique, digestif, infectieux, métabolique, hémodynamique, ophtalmologique et la croissance) entre le 1er janvier 2006 et le 31 décembre 2015. Résultats : Une différence significative est retrouvée lors de l’analyse de l’aide respiratoire durant la période néonatale entre le groupe étude et RCIU (p = 0,0116), mais aucune entre les autres groupes. Les autres items ne révèlent aucune différence significative lors de leur comparaison, sauf pour les troubles métaboliques qui sont plus fréquents dans le groupe RCIU. En conclusion, il ressort de cette étude que les jumeaux eutrophes pour lesquels la naissance a été déclenchée prématurément dans l’indication de souffrance fœtale de leur jumeau hypotrophe ont une évolution similaire aux jumeaux eutrophes issus de grossesses ne contenant pas de RCIU et de même âge gestationnel. Par contre, les sujets hypotrophes possèdent une meilleure évolution sur le plan respiratoire que les sujets eutrophes.
