This book explains how to use intravitreal steroids optimally in the management of patients with intraocular inflammation (uveitis) and macular edema. Het misbruik van endogene steroïden wordt opgespoord via steroïdprofilering waarvan de testosteron over epitestosteron (T/E) ratio de belangrijkste merker is. Uit een vorige steroïdprofileringsstudie uitgevoerd met GC-MS single quadrupool is gebleken dat er naast de T/E ratio ook andere ratio's bestaan die minstens even goede biomerkers zijn voor het opsporen van steroïdenmisbruik. Met de komst van het Atleet Steroïdaal Paspoort (ASP) en de perspectieven die deze nieuwe biomerkers bieden voor de verdere ontwikkeling van het ASP werd de inzetbaarheid van deze nieuwe biomerkers getest met het gevoeligere GC triple quadrupool en werd er een vergelijking gemaakt tussen de resultaten van beide methoden.Na de analyse van excretie-urines met GC-MS triple quadrupool werden de nieuwe biomerkers geëvalueerd op basis van hun relatieve respons en maximale detectietijd. De resultaten werden vervolgens vergeleken met de resultaten van single quadrupool.Uit de vergelijking kon vastgesteld worden dat de gevonden biomerkers bij single quadrupool en triple quadrupool vrij vergelijkbaar zijn. De ratio 6alfa-hydroxy-androstenedione/16alfa-hydroxydehydroepiandrosterone bevestigde zijn bruikbaarheid als biomerker met een langere detectietijd bij triple quadrupool na de orale toediening van testosteron.Het gebruik van de nieuwe biomerkers met GC-MS triple quadrupool kan zeker bijdragen tot de verdere ontwikkeling van het ASP mits er verder aandacht geschonken wordt aan de kwantificering van de minor steroïdmetabolieten.
