Gastroesophageal reflux disease (GERD) affects millions of people in western society. Since the introduction of laparoscopic techniques in the 1990s, it is now one of the most common indications for surgery. This multiauthored text provides information on the pathophysiology of GERD, appropriate medical management, and proper indications for and performance of surgical and endoscopic procedures for GERD. Thousands of patients will experience inadequate relief from medical therapy or will develop complications from their procedures or recurrent or new symptoms afterwards; this text describes in detail the proper evaluation and management of such patients. Beginning with an overview of the current understanding of the pathophysiology of GERD, the book moves onto new information regarding tissue susceptibility, the role of bile and digestive enzymes, and cyclic changes in LES tone. The next chapters are devoted to standard therapeutic options for GERD, including lifestyle changes, pharmacologic therapy, and surgery. In addition, newer endoscopic treatments, such as Stretta, Endocinch, injection of bulking agents, as well as others, are described. Subsequent chapters focus on the causes for failure of medical, endoscopic, and surgical therapies. The remainder of the text is a detailed examination of medical, endoscopic, and surgical remedies for these treatment failures. This text is essential to any physician who manages patients with GERD, including internists, gastroenterologists, general surgeons, and thoracic surgeons.

The increasing incidence of esophageal adenocarcinoma has created an enormous interest and stimulus for research in this area. GERD brings together, for the first time, a vast amount of disparate literature and documents the entire pathogenesis of reflux disease in one place. The book presents reflux carditis as a new diagnostic criterion of GERD and for the first time defines the dilated end-stage esophagus and the earliest microscopic phase of GERD that is missed by present diagnostic criteria. GERD presents both clinical and pathological information and is meant to be used as

Gastroesophageal reflux disease (GERD) is a very common, global clinical problem. It affects any age group, both males and females, and is seen mainly in developed countries, especially among obese individuals. GERD needs to be treated to prevent nuisance symptoms and long-term complications. The book deals with the diagnosis of GERD, including clinical presentations and diagnostic investigations, and describes the different available conservative, medical, surgical and endoscopic treatments. The book also covers gastroesophageal disease in children, its presentation and treatment. It also deals with the refractory type of gastroesophageal disease including different theories. It is very useful for gastroenterologists and upper gastrointestinal surgeons.

Gastroesophageal reflux disease affects many patients. This disease not only lowers their quality of life, but it also threatens some of them with an underhand risk of cancer. Additionally, it becomes an economic burden for the patients and society. The aim of this book on gastroesophageal reflux disease is to provide advice and guidance to gastroenterologists to help them understand and manage some aspects of this proteiform disease.

This unique book presents best evidence and practices from experts in the fields of gastroenterology, allergy, otolaryngology, surgery and pulmonology to best understand the current clinical challenges in this field and to help all subspecialties that interact with this group of patients. It outlines diagnostic and management strategies for each of the above symptoms so that we call all better care for this group of difficult to diagnose patients.

This book presents a new, multi- and interdisciplinary approach to gastroesophageal reflux disease (GERD) and Barrett’s esophagus that reflects the latest advances in understanding of the disease. The role of each of the available diagnostic techniques, including endoscopy, histopathology, esophageal manometry, reflux monitoring, and EndoFLIP assessment, is carefully explained. The use of endoscopy for the purposes of mucosal ablation (radiofrequency ablation, cryoablation) and mucosal resection is described, and all aspects of anti-reflux surgery are covered, with detailed explanation of indications and techniques for standard fundoplication, endoscopic fundoplication, and surgical procedures using the LINX system and the EndoStim device. To assist the reader further, a novel interdisciplinary algorithm is presented for GERD diagnosis and treatment. In addition, the latest diagnostic and therapeutic approaches (including endoscopic and surgical resection and chemoradiation) are also presented for esophageal cancer and cancer palliation. A key feature of the book is its recognition of the contribution of different specialties in the management of GERD, and it will accordingly appeal to a wide range of practitioners.
