Ce livre s'intéresse aux situations dans lesquelles le clinicien se confronte aux difficultés de la rencontre avec un sujet, ou un groupe de sujets qui manifestement souffrent sur le plan psychique, physique et/ou social, sans exprimer directement une demande. Elles couvrent un large champ d'expériences : des situations sociales et psychologiques d'extrême détresse, des situations de traumatisme collectif et de violence sociale, des situations de polyhandicap. Les souffrances psychiques évoquées se manifestent, directement ou indirectement, sur la scène sociale ; elles impliquent l'ensemble des acteurs du social. Les sujets concernés paraissent, au moins pour un temps, en position de ne pas pouvoir bénéficier des dispositifs de soins existants. L'enjeu pour le soignant est alors de déjouer la destructivité au cœur de ces situations, de se rendre accessible au patient afin d'engager une relation d'aide ou de soins, ou une rencontre thérapeutique. Les auteurs ont choisi d'aborder ici l'actualité des situations de détresse comme autant d'appels à la créativité dans les relations aux patients et dans les relations entre professionnels. En effet, recueillir les signes de souffrance du sujet, là où il les dépose, s'interroger sur ce qui vaut comme symptôme impliquent de déplacer son regard, d'accepter de changer de lieux d'exercice, d'instaurer des relations pluridisciplinaires et interinstitutionnelles, et surtout de penser autrement la souffrance de la personne.
