This volume reconstructs Carlo Livi (1823-1877)'s biography, focusing on his medical and psychiatric work and on his path as a man and intellectual who lived during the Italian Risorgimento. Livi was a key figure in Italian psychiatry, halfway between the therapeutic optimism of the early 19th century and the pessimism of post-Darwinian psychiatry, both in the practical aspects and in the theoretical elaboration. The study highlights how, in this case, the moral treatment applied by 19th century psychiatry throughout Europe is linked to the events of the Italian Risorgimento. The moralizing and educational request, as well as the anthropological study of the human being, were aimed at the sick, the insane, at children, at criminals and the Italian people in order to carry out a national and secular project of social regeneration in Italy.

Voici un récit de voyage en terre de psychiatrie, contrée semée d'embûches, de mensonges et de lâchetés « trop humaines » où se joue le vrai drame de la folie. Paul Risser nous conduit de ses débuts en centre hospitalier de conception asilaire, à l'avènement de la psychiatrie en secteurs, dans des établissements entièrement neufs. Les réformes se succèdent avec une réelle évolution des structures d'accueil, conçues de manière moderne et pour une autre forme de pratique. Mais bien des difficultés naissent ; les changements sont pensés sans tenir compte de leur mise en place dans les institutions, au quotidien et sur le terrain. Même l'architecture de ces nouveaux lieux de vie crée bien des problèmes tant aux soignants qu'aux patients. Et l'on constate que les bonnes intentions peuvent se transformer en conditions de travail infernales, et ne pas apporter d'amélioration aux conditions de vie des patients. Tout au long de son parcours professionnel, l'auteur, guidé par un attachement profond à la psychanalyse, s'est attaché avec beaucoup de conviction à faire évoluer les soins proposés aux patients. Son témoignage croise l'histoire de la psychiatrie strasbourgeoise et de ses acteurs (Théophile Kammerer, René Ebtinger, Lucien Israël notamment) et interroge le devenir de cette spécialité.
