Abstract

L’ostéochondrose est une maladie orthopédique de développement fréquemment décrite chez l’homme et chez plusieurs espèces d’animaux domestiques. Sa prévalence élevée au sein de la population équine a un impact économique non négligeable dans le domaine du sport, par ses répercussions cliniques, et dans le domaine de l’élevage, par la suspicion d’un facteur héréditaire. Cette pathologie est définie comme une non-ossification endochondrale des épiphyses pouvant mener à la formation de kystes et de fragments ostéo-chondraux (ostéochondrose dissécante). On sait aujourd’hui que la pathogénie repose sur une nécrose ischémique focale des chondrocytes au niveau de sites de prédilection bien définis. C’est une maladie à caractère dynamique puisqu’une régression et une guérison spontanée des lésions est possible durant la première année de vie. L’étiologie exacte de l’ostéochondrose reste à ce jour non élucidée mais il est admis qu’il s’agirait d’un problème complexe et multifactoriel impliquant des facteurs génétiques, environnementaux, endocriniens et biomécaniques. Un lien entre le développement de l’ostéochondrose chez le jeune cheval et ses conditions environnementales, comme l’alimentation, le type d’hébergement et le degrés d’exercice à des moments clés, comme la gestation et les premiers mois de vie, a clairement été établi. La croissance intra-utérine et en particulier le début de l’ossification endochondrale pourrait être modulée par la nutrition de la jument gestante, ce qui souligne le rôle du métabolisme énergétique dans l’étiologie de la maladie. De récentes études ont identifié plusieurs voies de signalisation cellulaire dont l’altération pourrait être liée à l’apparition de cette pathologie. Parmi elles, la voie signalétique Wnt/β-caténine semble être un régulateur majeur de l’activité osseuse et pourrait être directement reliée au développement de lésions d’ostéochondrose. La compréhension du rôle des différents facteurs environnementaux dans l’apparition de la maladie représente un enjeu de taille pour la mise en place de nouvelles stratégies de diagnostic et de prévention.

Abstract

Une place importante est occupée par l’ostéochondrose du grasset et les fractures de fatigue du carpe et du tarse dans les causes d’intolérance à l’effort et de réforme dans le milieu des courses. L’ostéochondrose est une pathologie développementale de l’os sous chondral suite à un défaut d’ossification endochondrale, alors que la fracture de fatigue est une pathologie dégénérative par surmenage de l’os sous chondral. L’utilisation de la radiographie permet de faire le diagnostic dans la majorité des cas, bien que pour les fractures de fatigue certaines vues doivent être répétées à divers angles afin d’observer la fracture. L’ostéochondrose du grasset ainsi que les fractures de fatigue de l’os carpal 3, de l’os tarsal central et de l’os tarsal 3 ont 2 options chirurgicales principales: l’ablation ou la fixation du « fragment ostéochondral ». La taille est le critère principal afin de décider du plan thérapeutique chirurgical. Outre ce critère, la qualité du « fragment ostéochondral » est un critère à considérer pour la décision de fixation d’une lésion d’ostéochondrose de la lèvre latérale de la trochlée fémorale. En général, il est recommandé de fixer les « fragments ostéochondraux » quand cela est réalisable afin de restaurer la surface articulaire pour les 2 entités pathologiques, soit l’aide d’implants résorbables pour l’ostéochondrose du grasset soit à l’aide de vis à compression pour les fractures de fatigue. Osteochondrosis of the stifle and stress fractures of the carpus and tarsus are important causes of poor performance and withdraw in the racing community. Osteochondrosis is a developmental pathology of the subchondral bone due to a defect in endochondral ossification, whereas a stress fracture is a degenerative pathology due to overuse of the subchondral bone. The use of X-rays allows the diagnosis to be made in the majority of cases, although for stress fractures certain views must be repeated at various angles in order to observe the fracture. Osteochondrosis of the stifle as well as stress fractures of the third carpal bone , central tarsal bone and third tarsal bone have 2 main surgical options: removal or fixation of the "osteochondral fragment". Size is the main criterion for deciding on the surgical treatment plan. In addition to this criterion, the quality of the "osteochondral fragment" is a criterion to be considered for the decision of fixation of an osteochondral lesion (osteochondrosis) of the lateral trochlear ridge of the femoral. In general, it is recommended to fix the "osteochondral fragments" when feasible to restore the articular surface for both pathological entities, either with resorbable implants for osteochondrosis of the stifle or with compression screws for stress fractures.

Abstract

Osteoporosis is one of the ten most important diseases worldwide and its importance will rise further f- lowing the aging of the population. Vertebral fractures are the most common fractures in osteoporosis and have signi?cant impact on quality of life and survival rates, as well as carrying increased socioeconomic costs. The establishment of minimally invasive treatment schemes in recent years has led to reduced peri- erative morbidity and increased early mobilization. This book focuses on balloon kyphoplasty and alter- tive minimally invasive treatments for stabilization of osteoporotic vertebral fractures. The optimal treatment of a patient with osteoporotic vertebral fractures requires an interdisciplinary - proach, and therefore our book draws on the expertise of orthopedic surgeons, traumatologists and neu- surgeons, and also includes a chapter on medical treatment. We are happy to present the ?rst English edition of this book. All chapters have been extensively revised and updated. In addition to a detailed description of the technique of balloon kyphoplasty, with practical tips from experts, the book retains a chapter on the medical treatment of osteoporosis, which is still ind- pensable in the interdisciplinary approach to osteoporosis.
