Opiate drugs are high-risks drugs, subjects to a high percentage of errors and adverse effects that can have serious or even fatal consequences on the health of treated patients. Therefore, special precautions must be taken to improve the safety and circulation of these medicines in hospitals. The pharmaco-therapeutic process is very complex and may include multiple errors in drug history, individualization of treatment, prescription of an order, transcription or interpretation of the prescription, preparation of the drug(s), delivery, administration, monitoring and therapeutic follow-up. Based on the literature and the answers collected by interviewing Belgian hospital pharmacists, we have identified numerous strategies and leads for improvement that could increase vigilance regarding the opiate circuit in hospitals. Les médicaments opiacés sont des médicaments à haut risque, sujets à un grand pourcentage d'erreurs et d'effets indésirables pouvant avoir des conséquences graves voire fatales sur la santé des patients traités. De ce fait, des précautions particulières doivent être prises afin d'améliorer la sécurité et le circuit de ces médicaments en milieu hospitalier. Le processus pharmaco-thérapeutique est très complexe et peut comprendre des multiples erreurs au niveau de l'historique médicamenteux, de l'individualisation du traitement, de la prescription d'une ordonnance, de la transcription ou l'interprétation de l'ordonnance, de la préparation du (des) médicament(s), de la délivrance, de l’administration, du monitoring ainsi que du suivi thérapeutique. Sur base de la littérature et des réponses collectées en interrogeant les pharmaciens hospitaliers belges, nous avons identifié des nombreuses stratégies et pistes d'amélioration qui pourraient accroitre la vigilance concernant le circuit d'opiacés en milieu hospitalier.
