Les gestes auto-dirigés visant " à se faire mal " peuvent être approchés de façon très diverse, en fonction de la personnalité et selon leur intensité, leurs conséquences, leur intentionnalité. Pour les formes mineures, nous avons retenu le terme " auto-offensif " et celui d'" auto-vulnérant " pour celles entraînant des lésions réversibles. Au geste responsable d'une amputation ou d'un dommage systémique significatif serait à réserver la désignation " auto-mutilation ". L'aspect répétitif a fait discuter l'opportunité d'individualiser un syndrome. La discussion étiopathogénique porte sur un déterminisme se situant au-delà du principe de plaisir, comme recherche d'une confirmation de soi, comme tentative d'endiguer les menaces de perte narcissique et les angoisses primitives suscitées par des confrontations au réel traumatisantes en l'absence d'espace de l'illusion, sans méconnaître, dans certains cas, la pesée de facteurs organiques.

Abstract

This book is the first account of self-harming behaviour in its proper historical and political context. The rise of self-cutting and overdosing in the 20th century is linked to the sweeping changes in mental and physical health, and wider political context. The welfare state, social work, Second World War, closure of the asylums, even the legalization of suicide, are all implicated in the prominence of self harm in Britain. The rise of 'overdosing as a cry for help' is linked to the integration of mental and physical healthcare, the NHS, and the change in the law on suicide and attempted suicide. The shift from overdosing to self-cutting as the most prominent 'self-damaging' behaviour is also explained, linked to changes in hospital organization and the wider rise of neoliberal politics. Appreciation of history and politics is vital to understanding the psychological concerns over these self-harming behaviours.

"Before discussing the central framing guiding this book (see Chapter 3), it is first important to provide foundational knowledge about self-injury. Accordingly, this chapter addresses key areas within the field of self-injury, including how self-injury is defined, who self-injures, what might contribute to self-injury engagement, the reasons people may have for self-injury, and the risks associated with engaging in self-injury. In Chapter 2, we build on this by discussing the link between self-injury and suicide.
