L'obésité, l'insulinorésistance et le diabète de type 2 sont fortement liés à une inflammation chromuque de faible intensité, accompagnée d'une production anormale de cytokines, d'une augmentation des protéines de phase aiguë, et d'une activation de cascades de signalisation pro­inflammatoires dans les tissus hépatique, musculaires et adipeux 1 • Les techniques classiques de mesure des paramètres impliqués dans les désordres métaboliques nécessitent beaucoup de temps, d'argent et de matériel biologique. Au cours de ce travail, nous nous sommes tournés vers une nouvelle technique, le MIA (multiplex immunoassay) pour mesurer les taux de cytokines circulants et de protéines impliquées dans la réponse à l'insuline au niveau du foie. L'avantage majeure de la technique MIA est la mesure simultanée de plusieurs analytes (jusqu'à 100) dans un volume faible et unique d'échantillon. Le but de ce mémoire fut double : (1)Implémenter cette technique au laboratoire. Deux expériences ont été réalisées chez des souris. Premièrement, nous avons évalué 6 protéines hépatiques répondant à l'insuline après une stimulation msulinique in vivo. Deuxièmement, nous avons déterminé l'impact du LPS, un stimulus pro­inflammatoire, sur plusieurs cytokines circulantes et différents marqueurs hépatiques impliqués dans la réponse à l'insuline. (2) Utiliser cette technique afin d'établir l'impact de désordres métaboliques d'origine nutritionnelle sur les taux de cytokines circulantes et sur les protéines impliquées dans la réponse à l'insuline. Les résultats confirment que la technique MIA permet de mesurer une hausse des cytokines plasmatiques (IL-la., IL-l p, IL-6, MCP-1, MIP-l a., TNF-a.) suite à une charge en LPS et que l'mjection d'insuline mène à la phosphorylation des protéines hépatiques suivantes : IR, IGF-lR, IRS- 1, p70S6K , Akt, GSK-3p. Chez des souris contrôles ou nourries par une diète hyperlipidique (HF) l>Clldant 4 semaines (modèle nutritionnel induisant une obésité et un diabète de type 2), la méthode s'avère parfois trop peu sensible que pour permettre la mesure de toutes les cytokines sélectionnées. Seul le taux d'IL- la. plasmatique est augmenté de manière significative chez les animaux recevant la diète HF, tandis que les autres cytokines ne sont pas modifiées. Cependant, un supplément de fructose chez des souris nourries par une diète HF pendant 8 semaines induit une augmentation de la concentration sérique de toutes les cytokines. Les protéines impliquées dans la réponse à l'insuline ne sont pas modifiées par le traitement nutritionnel malgré l'hyperinsulinémie, suggérant une résistance à l'insuline. Obesity, insulin resistance and type 2 diabetes are closely associated with chronic inflammation characterized by abnormal cytokine production, increased acute-phase reactants and other mediators, and activation of a network of inflammatory signaling pathways in the liver, the muscle and the adipose tissue 1• The classical technical approaches allowing to measure the key parameters involved in metabolic disturbances are time-, sample- and money-consuming.In the present work, we have investigated a new technique named MIA (Multiplex ImmunoAssay) to assess the level of cytokines and proteins involved in the insulin pathway. This technology enables simultaneous detection of up to 1OO proteins in the same well with a small sample volume. This work was devoted to two purposes :(1)To establish the MIA technology in our laboratory. Two experiments were performed on mice. First, we evaluated the proteins involved in insulin signaling in the liver after insulin stimulation in vivo. Second, we determined the impact of LPS, a proinflammatory stimulus, on serum cytokines and hepatic proteins reflecting insulin response. (2)To apply this technology to assess the impact of nutritionally induced metabolic disorders on cytokine levels and hepatic insulin pathway.The results confirmed that MIA technique allows to measure a huge increase in plasma cytokines (IL-la, IL-lp, IL-6,MCP-1, MIP-la, TNF-a) upon LPS challenge, and that insulin injection leads to the phosphorylation of the following hepatic proteins: IR, IGF-IR, IRS-1, p70S6K, Akt, GSK-3P. In mice fed a high fat diet (HF) during 4 weeks (nutritional model inducing obesity and type 2 diabetes), the method is sometimes not enough sensible to detect the chosen cytokines. Only plasma IL-1a was significantly increased in mice fed the HF diet, whereas the other cytokines were not modified. However, a fructose supplement in mice fed a HF diet for 8 weeks induces an increase of all the cytokine levels. Proteins involved in the insulin signaling were not modified by the nutritional treatment despite hyperinsulinemia, suggesting hepatic insulin resistance. In the future, the assessment of hepatic insulin cascade would require the analyzing of the phosphorylated proteins after insulin challenge, with the protocole established in the first part of this work.
