Le présent mémoire de recherche clinique a pour but d'analyser rétropectivement les dossiers de 45 patients atteints de gastrinome suivis à Sint-Pieter Ziekenhuis, UCL, puis aux cliniques universitaires Saint-LUC, UCL, depuis 1965 à aujourd'hui. Les 45 patients ont été divisés en 2 groupes ceux d'un gastrinome sporadique et ceux atteints d'un gastrinome associé à une néoplasie endocrinienne multiple de type 1 (NEM- 1) sy,drome héréditaire défini par Werner, associant une hyperparathyroïdie à une tumeur pituitaire. Nous analyserons les potentiels facteurs pronostiques en fonction de ces 2 catégories de patients. Par ailleurs, nous tenterons de définir le syndrome clinique de Zollinger-Ellison (SZE) après révision de la littérature. Ce syndrome avait été décrit par Zollinger et Ellison en 1955 et associait une diathèse ulcéreuse sévère, une élévation de la sécrétion acide et une tumeur pancréatique. Ces dernières années, le diagnostic de SZE est devenu de plus en plus difficile vu l'utilisation répandue des inhibiteurs de la pompe à proton (IPP) et un manque de fiabilité de certains kits de mesure de la gastrinémie. Nous définirons également les critères diagnostiques de gastrinome. Cependant, l'avènement des IPP, précédés par les anti-H2, aura considérablement modifié la prise en charge des patients avec SZE chez qui seule la solution pour contrôler la sécrétion acide était autrefois la gastrectomie totale avec une mortalité et une morbidité importantes. Une fois, la sécrétion acide contrôlée, la résection totale du gastrinome est essentielle à la survie des patients. Une bonne localisation de la tumeur primitive, en vue d'une chirurgie, peut être assurée par la combinaison de l'écho-endoscopie à la scintigraphie à l'octréotide. La ponction à l'aiguille fine réalisée à l'écho-endoscopie permet le diagnostic anatomopathologique de gastrinome. En outre, le recours à des techniques peropératoires peut être utile. La chirurgie dépendra de la nature du gastrinome et sera associée, si nécessaire, à des traitements médicaux tels que les analogues de la somatostatine associés ou non à l'interféron-α et la chimiothérapie à base de stretptozotocine - 5 fluorouracile (5-FU). Des thérapies plus récentes prometteuses font également leur apparition telles que leur apparition telles que la radiothérapie métabolique ou la thérapie moléculaire ciblée à base de sunitinib ou d'everolimus.Les traitements ablatifs seront surtout considérés dans une optique palliative.
