Abstract

Nausea is an unpleasant feeling. It can be followed by vomiting which a mechanism is allowing to evacuate the content of the upper part of gastrointestinal tube. Different molecules exist and act on different paths to inhibit those vomiting. Antidopaminergic molecules are frequently used as the domperidone. Domperidone antagonizes dopaminergic receptors which are found in the area postrema but also acts as a gastroprokinetic. Some studies reported a risk of QTc prolongation. This can lead to arrhythmias or sudden cardiac death. The risk of extra-pyramidal side effect exists but more often by children because of their blood-brain barrier which is not totally developed and by old people. Domperidone is also used to raise the production of milk for mothers who can’t produce enough to breastfeed their child. Indeed, secretion of prolactin rises up with domperidone. But this use is still off-label. More studies have to be done to prove the efficacy and the safety for mothers. La nausée est une sensation désagréable qui peut être suivie de vomissements. Ceux-ci permettent d'évacuer le contenu de la partie haute du tube digestif. Différentes molécules agissant sur différentes voies sont disponibles pour les inhiber. Les médicaments anti dopaminergiques sont fréquemment employés comme le dompéridone. Il agit en antagonisant les récepteurs dopaminergiques situés dans l'area postrema et possède également des propriétés gastroprocinétiques. Des études ont été réalisées sur cette molécule mettant en avant un risque d'allongement de l'intervalle QTc pouvant évoluer en torsades de pointe. Ces torsades de pointe sont susceptibles d'induire des syncopes ou peuvent dégénérer en fibrillation ventriculaire menant à un arrêt cardiaque ou provoquer la mort subite. Le dompéridone est aussi associé au risque de développer un syndrome extrapyramidal, plus particulièrement chez les enfants dont la barrière hémato-encéphalique n'est pas encore tout à fait développée et chez les personnes plus âgées. Comme dit précédemment, le dompéridone est utilisé dans le traitement des nausées et vomissements mais un usage en off-label est de plus en plus pratiqué. En effet, un de ses effets secondaires est d'augmenter la production de prolactine par l'antéhypophyse ce qui a pour conséquence une augmentation de la production de lait. Cette propriété est exploitée pour les mères ayant une production lactée insuffisante mais dont le bébé nécessite une alimentation par le lait maternel. Il a été évalué que seule une très petite quantité passait dans le lait maternel et qu'il n'y avait donc pas de danger pour le nouveau-né. Cependant, des études supplémentaires sont nécessaires afin de prouver l'efficacité de la molécule pour cette indication et pour évaluer sa sécurité chez la mère aux posologies prescrites.
