Das Hirsch-Institut für Tropenmedizin (HITM) wurde 2013 als Außenstelle der Klinik für Gastroenterologie, Hepatologie und Infektiologie der Heinrich-Heine-Universität Düsseldorf unter Leitung von Professor Dieter Häussinger eröffnet. Das HITM dient dem Zweck des Wissenstransfers in den Bereichen Wissenschaft und Medizin, der Ausbildung von Ärzten sowie der Durchführung wissenschaftlicher Projekte im Bereich der Infektions- und Tropenmedizin. Der vorliegende Band berichtet über die bisherigen Aktivitäten des Instituts, seine Historie und Perspektiven und erlaubt darüber hinaus auch einen Blick hinter die Kulissen des Aufbaus eines Instituts in Afrika und auf die Lebensumstände und Eindrücke der Mitarbeiter. Dieter Häussinger, Direktor des HITM und der Medizinischen Klinik und Poliklinik für Gastroenterologie, Hepatologie und Infektiologie und Lehrstuhlinhaber für Innere Medizin an der Heinrich-Heine-Universität Düsseldorf, ist im Bereich der klinischen und experimentellen Hepatologie und Gastroenterologie aktiv. Er engagiert sich für die Weiterentwicklung der klinischen Infektiologie. In diesem Zusammenhang erfolgte die Zertifizierung seiner Klinik als Zentrum für Infektiologie, der Aufbau einer tropenmedizinischen Ambulanz und Infektionssprechstunde, die Errichtung des Leber- und Infektionszentrums mit der einzigen Sonderisoliereinheit in Nordrhein-Westfalen für hochinfektiöse Patienten sowie die Gründung des Hirsch-Instituts für Tropenmedizin.

People who have chronic diseases spend a significant amount of time in self-management in out-of-hospital environments, in their homes and in their community settings. These patients have different disease statuses and management requirements, so providing personalized care is key.Personalized care is a collaborative process that should be used in chronic condition management in which patients, caregivers, and healthcare providers identify and discuss problems caused by or related to the patient's condition and then develop plans and goals to empower patients.Personalized care can improve aspects of physical health, mental health, and the ability to self-manage conditions. Thus, a personalized care approach could greatly benefit patients with chronic conditions.In this book, we have included 20 articles that expand the current knowledge on the personalized care approach to diagnosing and treating chronically ill patients.

With the development of analytical instruments, the academic system has become more complicated, producing new journals one after another. Therefore, it became much important to clarify what is original of “Medicines”. As the name Medicines indicates, it includes science and practice of caring for a patient and managing the diagnosis, prognosis, prevention, treatment or palliation of their injury or disease. Therefore, Medicines differs from other journals with similar title in that it covers a wide range of fields, from traditional medicine to rapidly developing molecular-targeted drugs, focusing on their pharmacological effects, structure–activity relationships, metabolic pathways, gene expression, using cultured cells, animals, and clinical trials. Most parts of this Special Issue were written by our editorial board members, who described the research topics and how they are engaged in their field of research. All of these articles are their life-long story. We collected a total number of 22 original works by basic researchers and clinical doctors. We hope that readers can get a chance to know the current status of diverse fields of medicines, and what your most important research themes are after reading these articles.

For a long time, the tight junction (TJ) was known to form and regulate the paracellular barrier between epithelia and endothelial cell sheets. Starting shortly after the discovery of the proteins forming the TJ—mainly the two families of claudins and TAMPs—several other functions have been discovered, a striking one being the surprising finding that some claudins form paracellular channels for small ions and/or water. This Special Issue includes 43 articles covering numerous dedicated topics including pathogens affecting the TJ barrier, TJ regulation via immune cells, the TJ as a therapeutic target, TJ and cell polarity, function and regulation by proteins of the tricellular TJ, TJ as a regulator of cellular processes, organ- and tissue-specific functions, TJ as sensors and reacting to environmental conditions, and last but not least, TJ proteins and cancer.
