Cholangiocarcinoma is a type of malignant tumor that affects the biliary system. It can arise anywhere in the bile ducts, manifesting itself as obstructive jaundice. It has an annual incidence of 1-2 cases per 100,000 people. One variety of the disease, the Klatskin tumor, occurs in the hepatic hilum and is characterized by poor prognosis. Generally, cholangiocarcinomas are slow-growing tumors with late metastases. Localized tumors are treated surgically, but intervention is discouraged when they are extensive. The disease affects both sexes, but most cases occur in patients aged over 60 years. Primary sclerosing cholangitis, chronic biliary infection, and choledochal cysts are associated with an increased risk of cholangiocarcinoma.

This book provides a thorough overview of the radiologic and pathologic findings of neoplastic diseases of the biliary tract, a sound knowledge of which is essential for appropriate patient management. A key feature is the correlation of imaging findings with both gross pathologic and histopathologic findings for each disease. This is of the utmost importance in understanding true pathologic abnormalities, bearing in mind, for example, the difficulty in ascertaining disease spread and extent on the basis of pathology alone. Since the first edition of the book, conceptual understanding of neoplastic disease of the bile ducts and cholangiocarcinogenesis has evolved significantly, and in the new edition the classification of diseases is based on the most recent edition of the WHO Classification of Tumors of the Digestive System. In addition, some newly recognized pathologies are covered. The book will enhance understanding of the pathology of neoplastic diseases of the biliary tract and aid imaging interpretation. It will be a great asset for pathologists, radiologists, biliary surgeons, and gastroenterologists. .

This volume discusses new developments in the etiology, diagnosis, and treatment of cholangiocarcinoma induced by the liver fluke Opisthorchis viverrini. Although cholangiocarcinoma is rare in the Western world, it has a very high incidence in continental Southeast Asian countries, where it is linked to infection with O. viverrini. In Southeast Asia this cancer causes thousands of deaths every year and places a severe socioeconomic burden on the affected families. Diagnosis is usually at a late disease stage, and in the past palliative surgery and care have been the major therapeutic options. Recently, however, techniques have become available for early-stage diagnosis that offer the possibility of curative surgery, and novel chemotherapeutic options are also under development. In Thailand alone, more than 2 million individuals have now been registered for screening for this cancer. The aim of this book is to highlight the latest research in the field and to explain how the newly available techniques can be applied to reduce the risk of developing cholangiocarcinoma and increase the likelihood of survival.

In this Special Issue of the journal, advancements in the treatment of liver diseases are illustrated by international experts in the field. New treatment options for primary biliary cirrhosis and, hopefully, primary sclerosing cholangitis are discussed. Up-to-date pharmacological therapy for preventing liver cirrhosis decompensation and treating acute-on-chronic liver failure is highlighted. Furthermore, new treatments for cholangiocarcinoma, based on biological and tissue markers, will be available in the near future, aiming to surpass the current unsatisfactory results of traditional therapies. Immunotherapy has been applied to hepatocellular carcinoma (HCC). The new first-line treatment, combining atezolizumab plus bevacizumab for HCC in the intermediate and advanced stages, will allow for an increase in patient survival in the near future. Liver transplantation (LT) remains the preferred treatment for many patients with end-stage liver diseases and HCC. The selection criteria for LT in patients with HCC moved from morphological to dynamic criteria, such as those derived from the assessment of tumor responses to locoregional and/or systemic treatments before transplantation. This allowed many patients who would have been excluded from a transplantation with the old selection criteria to access one. Finally, a very interesting issue regarding new indications for liver transplantation is illustrated.
