Vitamin D is well known for its role in the ossification and its use in the treatment against osteoporosis. Thanks to pleiotropic properties, it seems that vitamin D also plays a significant role the immune and inflammatory systems and more generally in the regulation of cellular proliferation. These new roles of vitamin D suggest a possible impact in pathologies such as autoimmune diseases, diabetes and cancer. Preventive or curative effects calcitriol (active form of vitamin D) supplementations in these pathologies are however not clearly demonstrated at this stage. This report aims to evaluate this issue in view of the latest publications. The implication of vitamin D in physiological processes such as regulation of glycaemia and inflammation or its impact on lymphocytes activation have been detailed aiming to better understand vitamin D multiple modes of action. Many factors influence these pathologies; therefore genetics has also been approached. We have also summarized data from the main clinical studies that have attempted to evaluate the role of vitamin D supplementation in cancer, diabetes and Crohn’s disease. In view of the important number of deficient people, the role of the pharmacist and the use of available forms are also described. La vitamine D est bien connue pour son rôle dans l'ossification et son utilisation dans l'ostéoporose. De par son action pléiotropique, elle semble jouer un rôle important dans la régulation des systèmes immunitaire et inflammatoire et de la prolifération cellulaire. Ces nouveaux rôles de la vitamine D laissent entrevoir un impact possible de cette vitamine dans des pathologies telles que les maladies auto-immunes, le diabète, ou le cancer. Le rôle du calcitriol (forme active de la vitamine D) dans ces pathologies ne semble pas toujours clairement démontré ni approuvé par l'ensemble des membres de la communauté scientifique. Ce mémoire a pour but de faire l'état de la question. L'implication de la vitamine D dans les processus physiologiques tels que la régulation de la glycémie et de l'inflammation autant que son impact sur les lymphocytes est détaillée de telle sorte à comprendre son mode d'action. Plusieurs facteurs influencent la survenue de ces maladies, c'est pourquoi la génétique est aussi abordée. Nous résumerons ensuite les données des principales études cliniques qui ont tenté d'évaluer le rôle d'une supplémentation en vitamine D dans le décours de la pathologie cancéreuse, du diabète ou de la maladie de Crohn. Au vu du nombre imposant de personnes en carence, le rôle du pharmacien d'officine et l'utilité des spécialités belges seront également décrits.
