Abstract

Con la diffusione delle TC Multidetettore la colografia virtuale (CTC) si sta affermando come nuova, affidabile metodica radiologica alternativa nella diagnosi della poliposi intestinale e dei tumori del colon-retto. La CTC è già oggi da considerarsi l’esame di prima scelta nella identificazione di lesioni tumorali sincrone in pazienti affetti da neoplasie del colon-retto non valicabili dall’endoscopio, fornendo simultaneamente una corretta stadiazione di malattia con evidenti benefici per la pianificazione terapeutica. La scarsa invasività rende la CTC una valida alternativa alla pancolonscopia anche nella ricerca dei polipi intestinali e costituisce la base per interessanti sviluppi in merito a future applicazioni a programmi di screening. Nel testo, rivolto ai radiologi che si avvicinano alla metodica, ai tecnici di radiologia ed ai medici coinvolti nell’iter diagnostico e terapeutico della patologia colo-rettale, vengono indicati i vantaggi della metodica, le sue indicazioni e la tecnica di esecuzione dell’esame con quadri iconografici dimostrativi di condizioni normali e patologiche. Seguono capitoli integrativi a carattere epidemiologico, chirurgico, anatomo-patologico e terapeutico affidati a collaboratori delle diverse branche specialistiche. Le conclusioni sono affidate al giudizio dell’endoscopista, insostituibile "alleato" ai fini del più corretto utilizzo clinico della CTC se intesa come non competitiva ma complementare ed integrativa della colonscopia tradizionale. anche nella ricerca dei polipi intestinali e costituisce la base per interessanti sviluppi in merito a future applicazioni a programmi di screening. Abstract

L'obiettivo di questo manuale è istruire il radiologo all'esecuzione della colonscopia virtuale, metodica che in pochi anni ha rivoluzionato la diagnostica per immagini del colon-retto. Oggi il test sta infatti rapidamente sostituendo il clisma opaco e in alcuni centri è utilizzato come esame di primo livello al posto della colonscopia, e non solo nei soggetti anziani e fragili. Studi clinici randomizzati ci diranno presto se la colonscopia virtuale avrà un ruolo nello screening del cancro del colon-retto.
