Portal vein thrombosis (PVT) has been considererd as an obstacle to liver transplantation (LT) Constantly evolution in the surgical techniques has demonstrated effectiveness in short and long-term survival. The aim of this study is to analyse all patients with portal vein trombosis transplanted at the university hospital Saint-Luc Brussels from 1984 to 1999. We retrospectively analyze the data from 110 adults with PVT at the time of LT. Because of a first report analyzing the initial experience in the surgical management and published in 1997, the patients were divided in two groups : those who underwent transplantation from January 1984 to December 1995 (group 1) and those transplanted from January 1996 to December 2009 (group 2). Venous trombectomy was performed on 29 patients in group 1 and 47 patients on group 2. Rethrombosis occurend in 3 patients (4,2 % patients, 3/71). Anastomosis of the graft's portal vein to the splenomesenteric confluence was realiized on four patients in group 1 and 0 in group 2. Three patients (75%) died on immediate postoperative course. We performed portal vein graft on eight patients in group 1 and on six patients on group 1. One patient (7,1% 1/14) presented a rethrombosis.None cavoportal hemitransposition was used on group 1 (9,7 L) were dramatically higher than in group 2 (1,1L) Early postoperative mortality (<3 months) were significantly but not statically (p=0,07) higher in the group 1 (23,26%) than in the group 2 (10,4%). This study shows clearly that changing the surgical approach to PVT can offer excellent for patients undergoing LT. La thrombose veineuse porte (PVT) était initialement considérée comme un obstacle à la transplantation hépatique (LT). Les évolutions continuelles dans les techniques chirurgicales ont démontré leur efficacité dans les survies à court terme et à long terme. Le but de cette étude est d'analyser tous les patients avec une thrombose veineuse porte à l'hôpital Universitaire de Saint-Luc UCL Bruxelles de 1984 à 2009 .Méthodes: Nous avons analysé de façon rétrospective les données de 110 adultes avec PVT au moment de la LT. A cause d'un premier rapport analysant l'expérience initiale dans le traitement chirurgical et publié en 1997, les patients ont été divisés en deux groupes: Ceux qui avaient reçu une transplantation de Janvier 1984 à Décembre 1995 (groupe 1) et ceux transplantés de Janvier 1996 à Décembre 2009 (groupe 2). Résultats: Une thrombectomie veineuse fut réalisée sur 29 patients dans le groupe 1 et sur 47 patients dans le groupe 2. Une rethrombose est survenue chez 3 patients (4,2%, 3/7 1). Une anastomose de la veine porte greffée au confluent spléno-mesentérique fut réalisée sur quatre patients dans le groupe 1 et sur 0 patient dans le groupe 2. Trois patients (75%) sont morts dans la période postopératoire immédiate. Nous avons réalisé une interposition par un greffon veineux sur huit patients dans le groupe 1 et sur six patients dans le groupe 2. Un patient (7,1%, 1114) a présenté une rethrombose. Aucune hémi-transposition cave-portale ne fut utilisée dans le groupe 1 mais fut utilisée sur quatre patients dans le groupe 2 avec une rethrombose (25%, lf4). Les besoins transfusionnels dans le groupe 1 (9,7L) étaient dramatiquement plus élevés que dans le groupe 2 (1,1L). Les taux de mortalité postopératoires étaient significativement mais non statistiquement (p=0,07) plus élevés dans le groupe 1 (23,26%) que dans le groupe 2 (10,4%). Conclusions: Cette étude montre clairement que les changements dans les procédures chirurgicales des PVT peuvent offrir des meilleurs résultats pour les patients recevant une transplantation hépatique.

According to Virchow's triad, venous thrombosis can occur as a result of one or more of three factors: changes in the dynamics of the blood flow, endothelial injury/dysfunction of the blood vessel and hypercoagulability. The blood in the veins is constantly forming microscopic thrombi that are routinely broken down by the body, and significant clotting can occur only when the balance of thrombus formation and resolution is altered. Central venous stenosis is a common complication arising from the use of tunneled central venous catheters. However, superior vena cava stenosis is a rare event, at least according to current litterature. Indeed, a thorough review brought up thirty-nine articles and among them thirty-five were case reports and case series. Only one study was able to corne up with an incidence (30%) after perforrning a transesophageal echography on twenty patients with a tunneled central venous catheter placed in the internat jugular vein.Superior vena cava stenosis can present with the typical « superior vena cava syndrome », through a catheter malfunction or be asymptomatic. This retrospective study was designed to study superior vena cava stenosis incidence during the year 2010 in four hemodialysis units. A hundred-and-fifty patients carrying a tunneled central venous catheter were recruited during this year and five of them suffered from a superior vena cava stenosis that was diagnosed after perforrning a phlebography, the current gold standard exam in central venous diseases. Unfortunately, the cases were found in only one unit and the number was too small to conduct a statistical analysis on the entirety of the data so only those from the unit containing the cases were used. The study downgrading from a multicentric to monocentric type is partly explained by several bias (detection, selection and survival). Nevertheless, this study shows that superior vena cava stenosis is not as uncommon as it seems since its incidence for a year and in only one unit was of 10,4%. Moreover, it's the first study giving an incidence value based on clinicat and dialysis events and not on systematic imaging (transesophageal echography, phlebography) , which is currently hard to perforrn on a broad population. Two risk factors, specific of superior vena cava stenosis, were detected : diabetes and young age (less than 60 year-old). However, the typical central venous stenosis risk factors such as the number of catheters carried or the duration of catheter carriage didn't statistically put patients at risk for superior vena cava stenosis in this study. A larger scale, preferably prospective study, with a special care to the bias found here, would be necessary to confirm these results. Les sténoses des veines centrales font partie des complications secondaires à l'utilisation des cathéters veineux centraux tunnelisés (CVCT) fréquemment rencontrées dans la pratique actuelle. Par contre, la sténose de la veine cave supérieure (VCS), reste un événement rare, tout au moins selon la littérature actuelle. En effet, Wle revue de celle-ci aboutit à 39 articles dont 35 case-reports ou case-series. Une seule étude rapporte Wle incidence (30%), après la réalisation d'une échographie transoesophagienne chez vingt patients porteur d'un CVCT jugulaire. Une sténose de la VCS peut se présenter avec une symptomatologie de « syndrome de la veine cave supérieure » ou par une dysfonction du CVCT ou même rester asymptomatique .Notre étude a eu pour but d'étudier rétrospectivement l'incidence des sténoses de la VCS durant l'année 2010 dans quatre centres d'hémodialyse hospitalière. Elle a permis de recruter 150 patients porteurs d'un CVCT durant l'année 2010. Chez cinq d'entre eux, une sténose de la VCS avait été diagnostiquée suite à l'apparition d'un syndrome clinique de la VCS ou suite à une dysfonction de CVCT et confirmée par phlébographie, le gold standard actuel. Malheureusement, le petit nombre de cas détectés et le fait que ceux-ci soient concentrés dans un seul centre n'a pas permis de réaliser une analyse statistique sur l'ensemble des centres. Le passage d'une étude multicentrique à monocentrique s'explique probablement par plusieurs biais (de détection, sélection et survie). Néanmoins, notre étude montre que la sténose de la VCS n'est pas aussi rare que la littérature ne le laissait penser puisque elle conclu à une incidence de 10,4% pour un an et cela dans un seul centre. De plus, elle est la première à montrer Wle incidence de sténose basée sur des évènements cliniques et/ou dialytiques et non pas sur la réalisation d'Wle imagerie systématique (échographie transoesophagienne, phlébographie), difficilement réalisable à grande échelle dans la pratique actuelle. Deux facteurs de risque, spécifiques des lésions de la VCS, ont pu être mis en évidence, ils'agit du diabète et du jeune âge (moins de 60 ans). Par contre, on ne retrouve pas les facteurs de risque classiques de sténose des veines centrales tels que le nombre de CVCT portés ou encore la durée de portage. Une étude de plus grande ampleur, idéalement prospective avec une attention particulière aux biais rencontrés dans notre étude serait nécessaire pour préciser ces résultats.
