Abstract

Research and clinical application of vitamin D has increased dramatically over the past decade stimulated by novel health promotion discoveries and documentation. This book brings together key researchers with their views focusing on the health promotion role of vitamin D. Such information is vital to clinicians, users of vitamin D supplements of all ages and those interested in public policy. The authors document and define many of the key health related roles of vitamin D. Its traditional application in bone and muscle health as well as therapy of arthritis is expanded and clarified with new research. A better understanding of the effects of vitamin D inadequacy is modelled using problems ranging from infant growth retardation to chronic kidney and periodontal disease. Uniquely the vitamin?s role in resistance and treatment of infectious diseases is shown in examples ranging from HIV/AIDS to tuberculosis. Mechanistic understanding of vitamin D's actions is enhanced by looking into its effects on immune modulation and inflammation. Expansion of the role of sunlight in stimulating vitamin D production is discussed relative to the reduction in a variety of cancers. Clearly vitamin D is like a two edged sword with great benefits but also some risks. This book provides carefully defined examples of both situations.

Abstract

The following chapters provide excellent and comprehensive discussions of the potential role of vitamin D based therapies in breast, colorectal, prostate cancer and leukemia and myelodysplastic syndromes. These chapters also point out that the focus on these diseases is largely determined by the interests and expertise of the outstanding scientists who have chosen to pursue vitamin D based cancer therapeutics. To our knowledge, every tumor type ever evaluated has shown some biochemical and antiproliferative response to vitamin D. Similarly, vitamin D analogues, especially calcitriol, potentiate almost every cytotoxic agent with which combination therapies have been tested.

Abstract

INTRODUCTION – L’insuffisance en vitamine D est un problème majeur de santé publique qui devrait être dépisté. A ce jour, le seul test fiable pour déterminer le statut en vitamine D est un test sanguin pour le dosage de la 25-hydroxyvitamine D, mais celui-ci ne doit pas être utilisé comme outil de dépistage, en particulier chez les sujets sains. L’objectif de cette étude est d'évaluer les performances de plusieurs questionnaires prédictifs simples, rapides et peu coûteux du statut en vitamine D et de déterminer s'ils peuvent constituer une alternative efficace au test sanguin dans la population générale.&#13;METHODOLOGIE – Il s'agit d'une étude transversale portant sur un échantillon de 81 sujets recrutés parmi le personnel du CHU et de l'Université de Liège, en Belgique (50,6°N). Tous les participants ont été invités à répondre à trois questionnaires visant à évaluer le statut en vitamine D. Un échantillon de sang a été prélevé et un dosage de la 25(OH)D sérique par spectrométrie de masse, en tant que référence, a été réalisé. Des paramètres tels que, la sensibilité, la spécificité, les valeurs prédictives positives et négatives ont été déterminés pour chaque questionnaire. Une analyse par courbe ROC a été entreprise. Des analyses de sensibilité ont été réalisées sur un sous-échantillon (n=40) composé de sujets non supplémentés uniquement.&#13;RESULTATS – 38,3% de l'échantillon total présentait une insuffisance en vitamine D avec un taux sérique de 25(OH)D inférieur à 20 ng/mL et 50,6% des sujets prenaient une supplémentation en vitamine D. L'âge médian était de 35,0 [28,0 - 43,0] ans et l'IMC moyen était de 24,4 ± 3,54 kg/m². Les analyses sur l'ensemble de l'échantillon ont montré que les questionnaires 1, 2 et 3 avaient une sensibilité de 87,1%, 83,9%, 100%, une spécificité de 78,0%, 20,0%, 4,00%, une valeur prédictive positive de 71,1%, 39,4%, 39,2% et une valeur prédictive négative de 90,7%, 66,7%, 100%, respectivement. Lors des analyses de sensibilité, il a été démontré que les questionnaires 1, 2 et 3 avaient une sensibilité de 96,4%, 82,1%, 100%, une spécificité de 8,93%, 16,7%, 0,00%, une valeur prédictive positive de 71,1%, 69,7%, 70,0% et une valeur prédictive négative de 50,0%, 28,6%, respectivement.&#13;CONCLUSION - Cette étude n’a pas pu démontrer une performance satisfaisante des questionnaires pour la prédiction du statut en vitamine D dans une population de sujets adultes en bonne santé.
