In this Master’s thesis we set out to explore the association existing between chronic spontaneous urticarial and thyroid auto-immunity. The first part of our work consists in a comprehensive review of all the available open scientific literature on the subject, whereas the second part details the results obtained by a retrospective clinical study led in the dermatology department at the cliniques universitaires Saint-Luc. Our Study spanned over a period of three years and included 65 patients. We chose to distribute the patients into two different groups: the cases are the 26,2% of the patients whose urticarial is associated with elevated levels of thyroid antibodies and the controls are the 73,8´% of the patients whose of the patients whose urticarial is not associated with thyroid auto-immunity. The data obtained were statically compared then put into perspectives with the input drawn from the literature review. By doing so, we found many similarities between the fruit of our work and numerous relevant publications on our subject. Our observations lead us to the conclusion that patients with thyroid auto-immunity in addition to chronic spontaneous urticarial could present a singularity of their immune system. However, the precise nature of this remarkable association of events has yet to be understood and further studies on this subject are required. Indeed, chronic spontaneous urticarial is a frequent although poorly known disease. The main mechanism resulting in the urticarial reaction is the cutaneous mast cells release of numerous mediators, predominantly histamine. However, this release seems to be triggered differently depending on the urticarial subtypes. It was suggested that an autoimmune process could be at work in over half all cases of chronic urticaria. This assumption is supported by the fact that thyroid auto-immunity is often observed in parallel with chronic spontaneous urticaria. The causes and consequences of this clinical association on the management and therapeutic care of our patient are still far from being fully elucidated. A direct pathogenic action of the thyroid antibodies has not yet been demonstrated and their presence is so far considered as a probable marker of latent autoimmunity. Our observations are in keeping with the current literature consensus recommending to spontaneous urticaria by reason of the high frequency of thyroid disorders in this subgroup of the population. Ce mémoire a pour but d'explorer le lien existant entre l'urticaire chronique spontanée et l'auto·immunité thyroïdienne. La première partie de notre travail consiste en une revue a littérature exhaustive des connaissances disponibles sur le sujet, tandis que la seconde partie expose les résultats d'une recherche clinique rétrospective menée au sein du service de dermatologie des cliniques universitaires Saint-Luc. Celle-ci s'est déroulée sur une période de trois ans et a rassemblé 65 individus. Notre série de patients est divisée en deux groupes : les cas sont les 26,2% des patients dont l'urticaire chronique spontanée est associée à des taux élevés d'anticorps antithyroïdiens et notre groupe témoin rassemble les 73,8% restant des patients dont l'urticaire chronique n'est pas associée à une auto-immunité thyroïdienne. Les caractéristiques cliniques des deux groupes ont été comparées statistiquement et les résultats obtenus ont été mis en perspective avec les données recueillies dans notre revue de littérature. Nous avons constaté de nombreuses similitudes entre le fruit de notre travail et les publications anciennes et récentes pertinentes à notre propos. Ces observations nous poussent à conclure à l'existence d'une singularité immunitaire chez les patients urticariens chroniques présentant de surcroît une auto-immunité thyroïdienne. Cependant la nature de ce lien doit encore être définie et d'autres études sont nécessaires à ce sujet. En effet, l'urticaire chronique spontanée est une pathologie fréquente mais encore peu comprise. Le mécanisme principal de la formation des papules urticariennes repose sur la libération de différents facteurs par les mastocytes cutanés, en particulier de l'histamine, cependant l'enchaînement des évènements y menant diffère d'un type d'urticaire à l'autre. Un processus auto­ immunitaire pourrait être à l'œuvre dans la pathogenèse de l'urticaire chronique spontanée d'un patient sur deux, cette hypothèse étant entre autre confortée par l'observation de l'existence fréquente de cette maladie en parallèle avec les pathologies auto-immunes thyroïdiennes. Les causes et les implications de cette association clinique sur la prise en charge et le traitement des patients urticariens sont encore loin d'être complètement élucidées.Un rôle pathogénique direct des anticorps antithyroïdiens sur la survenue de l'urticaire chronique n'a jusqu'à présent pas été mis en évidence, la présence de ceux-ci étant probablement un simple marqueur d'une auto-immunité latente. Nos observations s'alignent avec le consensus en vigueur dans la littérature reconnaissant l'utilité du dépistage des anomalies de la fonction thyroïdienne chez tous les patients souffrant d'urticaire chronique réfractaire au traitement antihistaminique, en raison de la fréquence élevée de dysfonctions thyroïdiennes dans cette population.
