Abstract

This book is focused on surgical treatment in trigeminal neuralgia (TN). Though microvascular decompression (MVD) is definite treatment for medically uncontrollable TN, manual focused on the surgical treatments are rare. This book includes overall view of TN, from diagnosis to treatment, more focused on technical pearls on MVD for neurosurgeons. Essential information of anatomy and pathophysiology are provided. Main topics would be indications, surgical considerations, electrophysiological monitoring during MVD surgery and outcomes of surgery. Furthermore, other non-surgical peripheral block techniques, radiosurgery and recent updated techniques will be discussed. The main target of this book will be neurosurgeons, neurosurgical trainees and also other physicians who’s in charge of patients suffering from neuralgic pains.

Abstract

The facialgia gather an important number of pathologies which the clinical presentation and therapeutic implications are various. Their physiopathology is not entirely known to date. The three most common facialgia are cluster headache, Horton's disease and trigeminal neuralgia. Their treatment depends from the chronicity and is made of different drugs like calcium channel blockers, triptans, anticonvulsivant drugs, ... The treatment is often specific to each algia. Regarding the forms refractory to drugs, some surgical options are possible although rarely performed Les algies faciales regroupent un nombre important de pathologies dont la présentation clinique et la prise en charge thérapeutique sont diverses. Leur physiopathologie n'est pas entièrement connue à ce jour. Les trois algies faciales les plus connues sont le cluster headache, l'artérite de Horton et la névralgie du trijumeau. Leur traitement dépend de leur forme (chronique ou épisodique) et est constitué de différentes classes médicamenteuses comme les antagonistes calciques, les triptans, les anti-épileptiques, ... Le traitement est souvent spécifique pour chaque algie faciale. En ce qui concerne les formes réfractaires au traitement médicamenteux, quelques options chirurgicales sont possibles bien que plus rarement réalisées
