Through the experimental study of fourteen cases, this research aims to verify first whether seven years postoperative adolescent with idiopathic scoliosis, show or not an alteration of gait kinematics, muscular mechanical work, and energy cost; and second whether, on the long-term, if the surgical curve's correction is preserved and the frontal body balance restored. These patients have already been evaluated for similar criteria in a previous study by a pre- and one year post-operative evaluation (published in 2010 by MAHAUDENS, P., et al.) [21]. We reassess seven years later the same cohort to compare the evolution of different parameters. About the effects of the spinal arthrodesis on gait, it was observed [21] an improvement in the gait kinematics and muscular mechanical work, one year after surgery. The energy cost of walking remained excessively elevate when comparing to healthy subjects. And we will analyze the radiological data to assess the evolution of the surgical correction and frontal body balance. The first goal of this search is to allow surgeons to provide better information's to the patients at the preoperative consultation, on the biomechanics and cosmetic evolution just after surgery and on the long term. The second goal of our research is to improve the quality of care given to the patients. This is also an opportunity to test and modify the technique of treatments to achieve to high accuracy therapeutic models, and at the same time allow the best quality of life for the patient. Grâce à l'étude expérimentale de quatorze cas, cette recherche tend d'abord à vérifier si sept ans après chirurgie, les adolescents souffrant de scoliose idiopathique montrent une modification de la cinématique de la marche, du travail mécanique, et du coût énergétique; et ensuite de savoir si, sur le long terme, la correction chirurgicale de la courbe scoliotique est préservée et l'équilibre frontal restauré. Ces patients ont déjà été évalués pour les mêmes critères dans une étude précédente évaluant ces paramètres en préopératoire et un an après l'intervention (publié en 2010 par MAHAUDENS, P., et al.) [21). Nous réévaluons sept ans plus tard, la même cohorte en comparant l'évolution des différents paramètres. Les résultats de l'étude concernant les effets de l'arthrodèse un an après la chirurgie ont monté une amélioration de la cinématique de la marche et du travail mécanique musculaire. Les données du coût énergétique restent excessivement élevées lorsqu'elles sont comparées à des sujets sains. En plus de ces facteurs, nous allons aussi analyser les données radiologiques pour évaluer l'évolution de la correction chirurgicale et de l'équilibre frontal.Le premier objectif de cette recherche est de permettre aux chirurgiens de fournir une meilleure information aux adolescents souffrant de scoliose idiopathique lors de la consultation préopératoire, sur la répercussion de la chirurgie sur la biomécanique de la marche et l'évolution esthétique juste après la chirurgie et sur le long terme. Le deuxième objectif de notre recherche est d'améliorer la qualité de soins donnée aux patients. Ce sera aussi l'occasion de tester et peut-être modifier les techniques de traitements pour obtenir des modèles thérapeutiques de haute précision, et ainsi d'améliorer la qualité de vie du patient.
