The new series A-Z Notes in Radiological Practice and Reporting provides practical guides for residents and general radiologists, organized alphabetically, primarily according to disease or condition. L’occlusione meccanica dell’intestino tenue rappresenta una problematica clinica di frequente riscontro, la cui gestione clinico-strumentale è resa difficile dalla complessità dei meccanismi responsabili del quadro occlusivo. Il ruolo del radiologo è quello di orientare tempestivamente l’iter terapeutico sulla base di una corretta interpretazione dei quadri iconografici. Lo scopo di questo volume –unico nel suo genere– è quello di correlare la fisiopatologia dell’occlusione all’imaging. La ricca iconografia, che spazia dall’esordio clinico alla risoluzione, enfatizza il ruolo della tomografia computerizzata multidetettore con mezzo di contrasto per via endovenosa, che è il gold standard attuale. Tuttavia, non vengono dimenticate le correlazioni con le metodiche eco-radiografiche, che conservano un efficace ruolo diagnostico, a basso costo biologico ed economico. L’ileo meccanico del tenue è un esempio eclatante a sostegno del sogno di molti addetti ai lavori: quello, cioè, di avere una figura di fine radiologo dell’emergenza addominale, esperto di organo e di malattia e non solo di macchina, come spesso purtroppo accade.

Small bowel obstruction is a common but difficult clinical problem in terms of appropriate diagnosis and timely management. In clinical practice, radiologists play a key role in the assessment of small bowel obstruction and can guide physicians toward appropriate and timely management of the many indeterminate cases and situations. However, to understand and connect the pathophysiology to the imaging features remains the most difficult task radiologists face in their clinical activity. The aim of this book is connect pathophysiology to imaging using many illustrations and examples, emphasizing the capabilities and limitations of multidetector computed tomography and its role in the correct management approach to these disorders. Furthermore, the place of allied imaging modalities (plain film and ultrasound) in the clinical algorithm is comprehensively illustrated.

The purpose of this book is to improve diagnostic yields of capsule endoscopy and double-balloon endoscopy, because those procedures can depict nonspecific findings that may not lead to a proper diagnosis. Another reason for the publication was recognition of the difficulty in distinguishing enteroscopic findings of ulcerative colitis from those of Crohn’s disease. From a practical point of view, it is important to observe endoscopic pictures first, then to compare the images of other modalities, and finally to compare macroscopic pictures of resected specimens. For that reason, a large number of well-depicted examples of small intestinal lesions were assembled to clarify differences among small intestinal lesions that appear to exhibit similar findings and morphologies. Comparisons with radiographic findings comprise another important element in diagnosis. There are limitations in endoscopic observations of gross lesions of the small intestine, with its many convolutions. In Japan, many institutions still practice double-contrast imaging, which provides beautiful results. Because a single disorder may exhibit variations, this volume includes multiple depictions of the same disorders. Also included are lesions in active and inactive phases, as both appearances are highly likely to be encountered simultaneously in clinical practice. The number of illustrated findings therefore has been limited to strictly selected cases.

In Japan, cadaveric donor liver transplantation is not common even though cadaveric organ transplantation was legally established in 1998. In contrast, the number of living donor liver transplantations is increasing, with more than 1700 cases at 43 Japanese institutes by November 2001. Indications for and have become living donor liver transplantation are widening in Japan similar to those for cadaveric donor liver transplantation in the United States and Europe. At the same time, split liver transplantation from cadaveric donors shares some technical aspects with living donor liver transplantation. Remarkable progress has been reported recently, and thus it was an auspicious time to hold a symposium on "Current issues in liver/small bowel transplantation" in Japan. We were honored to hold a very fruitful symposium sponsored by the Keio University Medical Science Fund and to bring together top-rank transplant surgeons from Japan and other countries. It was a productive and rewarding time for all participants. We were able to share our experience through excellent presentations followed by active discussions and insightful com ments. At the symposium, we focused on current issues in liver transplanta tion such as widening indications for viral hepatitis and malignant tumors. We also discussed technical aspects and physiological problems in split/iiving donor liver transplant, novel strategies in immunosuppression, and the current status and future prospects in small bowel transplantation. This book contains the papers from all the distinguished guest speakers, focusing on the topics discussed at the symposium.
