Introduction : En Europe, 20 à 30% des accidents de véhicules motorisés sont liés à la somnolence. La proportion de travailleurs de nuit est estimée à environ 19% en Europe. Le travail de nuit est responsable d'une augmentation de la somnolence. Le but de cette revue de littérature est donc d'évaluer l'impact du travail de nuit sur le risque d'accident ou de presque accident. Méthodologie : Les bases de données Cochrane Library, Cinahl, Embase, PubMed ont été consultées pour la période qui va du 1/01/1997 au 9/0712017. La recherche a été basée sur le lien entre le travail de nuit et les outcomes principaux : le risque d'accident ou presque accident ainsi que les mesures prédictives de ce risque. Résultats : Sur 680 titres parcourus, 7 études ont été retenues dont 4 études en crossover sur simulateur ou sur circuit fermé. Les 3 autres ont été réalisées au moyen d'interviews, journaux de bord et questionnaires auto-rapportés. Les 7 études ont montré une augmentation significative du risque d'au moins un des outcomes principaux. Conclusion : Le travail de nuit augmente le risque d'accident ou de presque accident durant le trajet travail-domicile.Ces résultats sont à prendre avec précaution au vu de la qualité méthodologique et du design des études incluses dans la recherche.

Le sommeil diurne du travailleur nocturne : étude de la variation quotidienne de son architecture par mesures répétées. Il s'agit d'un mémoire-recherche qui s'intéresse à l'influence de l'inversion circadienne du rythme veille/sommeil sur la structure du sommeil, chez les travailleurs de nuit. Objectif : Etablir les différences dans l'architecture du sommeil diurne par rapport au sommeil nocturne et l'évolution journalière, principalement pour la distribution des sommeils lent profond (SLP) et paradoxal (SP) Méthodes : Enregistrement de 5 épisodes de sommeil : 1 nuit (J0), en période de rythme veille/sommeil classique, et 4 jours consécutifs (J1 à J4) pendant le travail de nuit. Ce schéma a été répété 4 fois (session 1 à 4) chez 8 infirmières. Nous avons, entre autres, évalué la période totale de sommeil (PTS), l'efficience de sommeil, les latences d'endormissement et de SP,l a durée des stades 1, SLP et SP, la distribution du SLP et du SP au cours du sommeil. Les variables ont été comparées entre J0 et J1 (différence) et de J1 à J4 (pente).Pour l'analyse des distributions des SLP et SP, nous avons découpé les épisodes de sommeil en « tiers » et y avons relevé la présence de chacun des stades avant d'en comparer la tendance de chaque stade au cours d'un épisode de sommeil. Résultats : La plupart des « différences » sont significatives : la PTS diminue de 150 ± 56, la quantité de stade 1 diminue de 4 ± 3 minutes, la proportion de SLP augmente de 12 ± 5 % du TTS et la quantité de SP diminue de 49 ± 34 minutes. Pour les « pentes », seule celle de la quantité du SP est significative, il augmente de 5±4 minutes par jour. La distribution du SLP est plus stable que celle du SP, mais les deux varient au cours des sessions. Le SLP s'homogénéise et le SP va jusqu'à inverser sa distribution en fin de session certaines fois.
