This thesis is intended for those interested in knowing more about the treatment of lower back disorders and more particularly about two specific pathologies i.e. lumbar spinal stenosis and chronic pain syndrome. This work is based both on review of available medical litterature relying on the principle of « Evidence-Based Medicine »and on a study of clinical cases carried out in Neurology and Algology of St Luc University Hospital. Due to their increased prevalence and their significant socio-economic impact lower back problems are an important public heath issue. Moreover, since this condition has a tendency of becoming chronic, it is essential that the most suitable treatement be developed as quickly as possible. Nevertheless, the current treatments of lumbar spinal sténosis due to arthrosis are still very much debated especially with regard to persisting low back pain. With increased ageing of the population lumbar spinal stenosis become more and more frequent although .The imaging techniques witch are available today enable to diagnosis the pathology more easily. The treatment of chronic low back pain syndrom (most frequently after surgery) is very difficult. We may rarely find a definite solution or an effective treatment Le présent travail s’adresse à ceux qui souhaitent en savoir plus sur le traitement de la lombalgie dans le cadre de deux pathologies spécifiques qui sont le canal lombaire étroit et le syndrome douloureux chronique. Basée sur une revue de la littérature médicale qui utilise le principe de l’Evidence Based Médicine et une étude de cas cliniques dans les services de Neurologie et d’Algologie aux Cliniques Universitaire St. Luc. La lombalgie constitue un problème de santé publique énorme de par sa fréquence et les répercussions socio-économiques qu’elles engendrent et cela est encore plus vrai quand le phénomène se chronicise. C’est pourquoi il est essentiel, que le traitement soit le plus approprié et le plus rapide possible. Le résultat du traitement du canal lombaire étroit arthrosique (souvent chirurgical ) est controversé, surtout en ce qui concerne la persistance de la lombalgie. C’est une pathologie en augmentation du fait du vieillissement de la population et de la détection par l’imagerie, qui est de plus en plus performante. En ce qui concerne le traitement de la lombalgie chronique post-chirurgicale, est très difficile et au lieu de parler d’arbre décisionnel, on pourrait parler d’une ‘" boucle décisionnelle " car c’est rare que l’on parvienne à une solution tout à fait satisfaisante.
