This authoritative clinical reference provides comprehensive coverage of all aspects of rheumatoid arthritis. The basics of rheumatoid arthritis are thoroughly covered in order to provide a firm foundation for the main focus of the text: therapy and clinical management of the disease. Practitioners will find detailed information on both pharmacologic and nonpharmacologic courses of management, with special emphasis on the management of pain. More than 100 full-color illustrations provide clear visual support for the concepts in the text. Online references in each chapter, as well as an entire chapter on Web-based information resources, keep this book on the cutting edge of this rapidly evolving field.

The pathogenesis of rheumatoid arthritis (RA) is incompletely understood. HLA class II alleles and T cells have been implicated for many years. The discovery of anticitrullinated peptide antibodies (ACPAs), along with the effectiveness of biological treatments targeting cytokines, such as TNF-?, IL-6, and also T cells and B cells, reinforced the pathogenetic role of the respective factors. ACPAs, induced by cigarette smoking and periodontitis in individuals with HLA-DRB1 shared epitope, appear to be autoantigens that initiate the inflammatory immune response in RA. MicroRNAs, part of epigenetic mechanisms, which also include DNA methylation, and histone modification, as well as microbiota, the composition of microbes in body cavities, also appear to influence arthritis and are discussed in this book.

Rheumatoid arthritis is a bewilderingly complex disease involving the interactions of many, and varied, cell populations and multiple families of low and high molecular mass mediators. We are only slowly beginning to understand the mechanisms that produce the local and systematic pathology clinically recognized as rheumatoid arthritis. Increasingly, use is being made of experimental models of this disease in an effort to test hypotheses about putative pathological mechanisms and to investigate the effect of novel therapeutic agents. A major section of this book covers these experimental models

Mary Felstiner went to bed one night an active professional and healthy young mother, and woke the next morning literally out of joint. With wrists and elbows no longer working right, she'd discovered one of the first signs of rheumatoid arthritis, the most virulent form of a common disease. While arthritis pain affects one out of three Americans, this book is the first to tell the personal story of the nation's most common yet neglected disease. Part memoir, part medical and social history, Out of Joint folds the author's private experience into far-reaching investigations of a socially hidden ailment and of any chronic condition. Moving from the 1940s to the present, this story of one life with arthritis exposes little-known medical research and provocative social issues: controversies over miracle drugs, demands concerning disability, and the surprising and disproportionate number of women affected by chronic illness.--From publisher description.
