Abstract

In the United States, respiratory failure accounts for 1275 cases per 100,000 people. The etiology of respiratory failure can be diverse, encompassing obstructive and restrictive lung diseases, among others. The COVID-19 pandemic led to a significant surge in respiratory failure cases among patients admitted to hospitals, particularly those requiring intensive care. This book explores the pathophysiology of respiratory failure and insufficiency associated with COVID-19 illness as well as obstructive lung disease. It also delves into respiratory insufficiency stemming from liver disease and burns and discusses the role of veno-venous extracorporeal membrane oxygenation (VV-ECMO) in managing patients with respiratory insufficiency. Every year, several specialists from all over the national territory take part to the Master. This work is a collection of all the dissertations presented by the attendants of the seventh edition of the Master in Interventional Pulmonology of the academic year 2015/2016, and acts as evidence and worthy conclusion for a demanding but fruitful year of study and practical activities aimed at certifying professional competence, which can be useful and expendable in the curriculum of a pulmonologist.

The textbook "Pulmonary Hypertension - From Bench Research to Clinical Challenges" addresses the following topics: structure and function of the normal pulmonary vasculature; disregulated cellular pathways seen in experimental and human pulmonary hypertension; clinical aspects of pulmonary hypertension in general; presentation of several specific forms of pulmonary hypertension, and management of pulmonary hypertension in special circumstances. The textbook is unique in that it combines pulmonary and cardiac physiology and pathophysiology with clinical aspects of the disease. First two sections are reserved for the basic knowledge and the recent discoveries related to structure and cellular function of the pulmonary vasculature. The chapters also describe disregulated pathways known to be affected in pulmonary hypertension. A special section deals with the effects of hypoxia on the pulmonary vasculature and the myocardium. Other three sections introduce the methods of evaluating pulmonary hypertension to the reader. The chapters present several forms of pulmonary hypertension which are particularly challenging in clinical practice (such as pulmonary arterial hypertension associated with systemic sclerosis), and lastly, they address special considerations regarding management of pulmonary hypertension in certain clinical scenarios such as pulmonary hypertension in the critically ill.

Abstract

Sarcoidosis is a type of inflammation that occurs in various locations of the body for no known reason. Normally, when foreign substances or organisms enter the body, the immune system will fight back by activating an immune response. Inflammation is a normal part of this immune response, but it should subside once the foreign antigen is gone. In sarcoidosis, the inflammation persists, and some of the immune cells form abnormal clumps of tissue called granulomas. The disease can affect any organ in the body, but it is most likely to occur in the lungs. It can also affect the skin, eyes, liver, or lymph nodes. Although the cause of sarcoidosis is not known, research suggests that it may be due to an extreme immune response or extreme sensitivity to certain substances. It also seems to have a genetic component as well, and tends to run in families. Sarcoidosis most commonly develops in people between 20 and 50 years of age. African Americans are somewhat more likely to develop sarcoidosis than Caucasians, and females are somewhat more likely to develop sarcoidosis than males. The symptoms of sarcoidosis depend on the organ involved. This book deals with the diagnosis and treatment of this mysterious disease of unknown etiology.
