"In this atlas, the authors (European and American specialists) demonstrate the value of thoracoscopy and provide the details necessary for the novice by combining textbook and atlas formats. This book encompasses the history of thoracoscopy, as well as its technical characteristics and the morphological appearance of pathologic tissues, based on the authors' personal experiences and on the literature. The authors emphasize those technical areas where thoracoscopy differ from other types of endoscopy. Photographs are supplemented by case reports which illustrate the indications, techniques, and outcome of the procedure. The book is organized according to clinical criteria on which the physician can base his or her decision to perform thoracoscopy. Because of the variability of presentation, common diseases are shown in several manifestations, whereas more rare diseases are presented only when their thoracoscopic appearance is characteristic. The incidence of pleural disease is increasing, and it is now accepted that best practice requires the incorporation of thoracoscopy into the work-up. However, surgeons cannot meet the rising demand for thoracoscopy by means of video-assisted thoracoscopic surgery (VATS), and waiting lists for endoscopic pleural procedures are consequently lengthening. Against this background, there is an increasing need for medical thoracoscopy performed by pulmonologists, which may be video assisted and has the advantage of being very cost-effective. In the age of the World Wide Web, informed patients continue to surprise oncologists with detailed questions about popular tumor therapies. Although minimally invasive tumor therapies (MITT) have become daily clinical practice for palliative treatment of liver tumors, the acceptance of these palliative modalities still varies enormously. In addition, promising results have been published for in situ ablation of lung metastases, bone tumors, and renal cell carcinoma.
