L’hypothèse selon laquelle des patients peuvent bénéficier du fait d’avoir des lésions carieuses occlusales, avec pénétrations dentinaires limitées, scellées plutôt que restaurées a été testé dans le projet Seal-B dans lequel ce mémoire s’inscrit. L’essai clinique randomisé a évalué et comparé deux stratégies de contrôle de progression de la carie occlusale utilisées dans la pratique quotidienne.15 patients âgés entre 18 et 30 ans à l’entrée de l’étude ont pu être contrôlés après 3 ans. L’examen clinique de ces patients avait mis en évidence au moins deux dents avec des caries occlusales actives (non-cavitaires ou cavitaires) et les radiographies bitewing ont montré que celles-ci avaient pénétré dans la dentine. Le tirage au sort des traitements accordés aux dents sélectionnées a permis de définir de manière aléatoire celle qui allait être traitée par le traitement non-opératoire (sealant) et celle qui allait bénéficier d’un traitement opératoire (restauration composite). Les résultats obtenus lors de la présente étude après une période de suivi de 3 ans montrent clairement que le scellement peut être une option de traitements des lésions occlusales avec pénétration dentaire limitée. Cliniquement, les sealants ont tous démontrés une fiabilité satisfaisante et similaire à celles des restaurations classiques. Par voie radiographique, quelques cas de régression ont été observés pour des dents scellées. À ce stade de l’étude Seal-B, le nombre de patients est encore trop limité pour permettre des conclusions définitives. Il est donc important de continuer le travail déjà entrepris pour atteindre l’objectif prévu de 50 patients possédant deux dents traitées de manière différente , d’une part par un sealant et d’autre part avec une obturation classique, et suivis sur une période de trois ans.

This book describes the challenges that deep carious lesions pose for dental practitioners, including the risk of endodontic complications and the difficulty of restorative treatment, and identifies options for overcoming these challenges on the basis of the best available evidence.
