Le cancer est un problème majeur de santé publique responsable annuellement de millions de décès. Michael Retsky a été le premier scientifique à s'interroger sur la croissance non linéaire des tumeurs montrant des périodes de dormance et d'accélération de croissance. Le risque de rechute en fonction du temps évolue selon un modèle en double pic après la résection avec un 1er pic précoce les deux premières années post-opératoires suivi d'un 2ème pic. La chirurgie est associée à un état inflammatoire qui semblerait promouv oir les métastases par la dissémination des cellules tumorales. De nouveaux outils périopératoires qui pourraient influencer le 1er pic ont été développés, tels que 1'administration d'une dose unique intraopératoire d'un AINS, le kétorolac, par l 'anesthésiologiste lors de la chirurgie cancéreuse. Une analyse d'imputabilité a été conduite afin d'évaluer l 'effet anticancéreux du kétorolac selon la méthode française, la méthode de !'OMS, la méthode de Kramer, la méthode de Naranjo et la méthode d'Emanueli. Les premières études conduites à l'UCL (Forget P., Machiels J .P. et al., 2013 ; Forget P., Bentin C. et al., 2014) suggèrent que l'effet anticancéreux concernant l'augmentation de la survie globale ou OS et de la survie sans maladie ou DFS est « douteux » à « probable » au niveau des cancers mammaires. L'effet anticancéreux du kétorolac sur la survie globale ou OS est « douteux » à « probable » au niveau des cancers pulmonaires non à petites cellules. Cancer is a major public health problem responsible for millions deaths annually. Michael Retsky was the first scientific to investigate the nonlinear tumor growth, exhibiting alternating periods of dormancy and accelerated growth. The risk of relapse as a function of time evolves according to a bimodal model after the resection with a first early peak during the first two postoperative years followed by a later peak. Surgery is associated with an inflammatory state which seems to promote metastasis by tumor cell dissemination. New perioperative tools were developed such as intra-operatively single dose administration of NSAID, the ketorolac, by the anesthesiologist during the cancer surgery and which could influence the first peak. An analysis of imputability was conducted according to the French method, the WHO method, the method of Kramer, the method of Naranjo and the method of Emanueli. First studies realized in UCL (Forget P, Machiels J.P, et al., 2013 ; Forget P., Bentin C. et al., 2014) suggest that the anticancer effect which is an increase of overall survival and disease-free survival is “doubtful” to “probable” for breast cancer. The anticancer effect of ketorolac for non-small-cell lung carcinoma concerning the overall survival is doubtful” to “probable”.
