Abstract

La contention, ou plutôt les systèmes de contention sont des dispositifs permettant de maintenir les dents dans une position déterminée, proche de la position physiologique. Ils sont indiqués lors de nombreux traitements parodontaux, orthodontiques, ou ortho-parodontaux. Ces contentions font partie intégrante de la phase active de ces traitements. Leur conception, leur réalisation et leurs indications, que la méthode soit directe ou indirecte, sont conditionnées par de nombreux paramètres : mobilités résiduelles, morphologie coronaire des dents, nombre de dents à contenir, choix des matériaux, motivation et hygiène du patient. Les contentions par méthode directe présentent l'avantage d'être réalisées en une séance et sont le moyen de contention le plus économe sur les plans tissulaire et financier. Leur réalisation est parfois délicate, leur longévité relative et, elles ne supportent que de faibles contraintes. Les contentions par méthode indirecte sont techniquement et technologiquement plus complexes ; elles sont réalisées par un laboratoire de prothèse. Leur longévité est grande et, elles peuvent supporter des contraintes mécaniques importantes. L'objectif de ce guide clinique est d'énoncer les différentes techniques de contention admises aujourd'hui, d'en définir les indications et d'en décrire les protocoles.

En tant que membre du comité de rédaction du Journal of Aligner Orthodontics et membre de la faculté Align Tech en Amérique du Nord, elle participe à plusieurs projets de recherche et à des essais cliniques de la FDA sur les aligneurs. Le Dr Tai est actuellement professeur adjoint d'ortho- dontie et coordonnateur du programme Invisalign à l'université de la Colombie-Britannique à Vancouver. Elle présente également un programme de conférences internationales s'adressant à des orthodontistes du monde entier sur le sujet des aligneurs et de leur potentiel. Le Dr Tai est membre du Collège des chirurgiens-dentistes de Colombie-Britannique et du Collège royal des dentistes du Canada, ainsi que membre de plusieurs autres organisations professionnelles. Elle est membre fondatrice et ancienne présidente du Orthodontic Ties Seminars Study Club à Vancouver et du Vancouver Invisalign Study Club, où elle encadre et forme les dentistes au système Invisalign. Elle a reçu sa formation en orthodontie à l'université du Minnesota (Etats-Unis)."

The relationship of oral disease to overall disease is certainly not a new concept. For centuries, the role of oral infection and inflammation in contributing to diseases elsewhere in the body has been studied and reported. During the last few decades, a series of intriguing reports from many countries have increased the current interest in the role of oral health and disease in contributing to general health and systemic conditions. Is it possible that oral and periodontal disease is a risk factor for cardiovascular and other systemic diseases? Since this question was first posed, a phenomenal body of work has been directed at understanding how oral periodontal disease might affect distant sites and organs and, thus, have an effect on overall health. Recent studies of the human microbiome using DNA sequencing technologies have revealed new insights into the possible mechanisms that help to explain how oral infections can occur in distinct sites such as atheromas, the colon, and reproductive tissues.
