Aims: The aims of this thesis are (1) to analyze a new psychophysical method for assessing intranasal trigeminal function and (2) to evaluate the usefulness of this method when applied to data on patients with an olfactory impairment. Method: In this study, participants were divided in accordance with the etiology of their altered olfactory function. For a total of 26 participants in the study: there were 8 patients with a post-infectious etiology (average age: 51.38 ± 17.78, 7 women), 4 post-traumatic patients (average age: 53.75 ± 9.81, 1 woman), 3 patients with chronic sinusitis (average age: 46.33 ± 14.50, 1 woman), 8 idiopathic patients (average age: 63.25 ± 14.18, 3 women) and 3 patients with another etiology (average age: 62.25 ± 16.80, 3 women). There was no significant difference between the different groups with regard to age (p=0.394) and gender (X/2 = 8.929, p= 0.063).In order to be able to compare the population studied with a control group, we compared the results of our cohort with a selection of controlled cases (age and gender-matched) from the study conducted by Doctor Huart (ENT at Saint-Luc). For the controls, we were able to use the values from the trigeminal test. The Sniffin' Sticks Test, was not given to the control group. We recorded the olfactory scores of al!the participants in our study by means of the "Sniffin' Sticks test". These assess several aspects of olfactory function: threshold, discrimination and identification. This was done by means of unirhinal testing. We also assessed their trigeminal function by means of a psychophysical test which is analogous to the "Sniffin' Sticks Tests". This psychophysical test in question is the "Trigeminal Sticks Test" which was developed, among others, by Dr. C. Huart and Professor. Ph. Rombaux. It uses the same principles as the "Sniffin' Sticks Tests". Thus we measured threshold, discrimination and identification. These measurements we conducted by means of birhinal testing but we added lateralized testing as an additional component. Conclusion: (1)The test is therefore suitable for assessing trigeminal function but only in in accordance with age: the older the patient is, the lower the test scores are. On the other hand, in our pathological group, neither the gender factor, the smoking status of the individual nor the presence or absence of parosmia or phantosmia, had any impact on the results while in a healthy group the gender factor was seen to be important.(2)When post-infectious and post-traumatic etiologies as well as chronic sinusitis were considered, older individuals performed Jess well in olfactory tests than younger ones which was not the case for "idiopathic" patients. Whatever the etiology, there were olfactory and trigeminal impairments, but the latter was not shown to have a particular pattern that corresponded with the type of etiology. The trigeminal test has therefore proved to be of interest for routine clinical use in determining the extent of impairment to trigeminal function and therefore the ability or lack of ability to maintain its sentinel function for the respiratory tract. On the other hand, there are no reproducible systems for confirming the type of etiology responsible for the damage. There was no increase in olfactory symmetry in accordance with the different types of etiology. Objectifs : Les buts de ce mémoire sont (1) d'analyser une nouvelle méthode psychophysique développée pour l'évaluation de la fonction trigéminale intranasale et (2) d'évaluer l'utilité de la méthode lors de la réalisation de bilans pour des patients présentant des troubles olfactifs. Méthode : Les participants à cette étude ont été répartis selon l'étiologie de l'altération de leurs fonctions olfactives. Nous comptons ainsi un total de 26 participants dont : 8 patients avec une étiologie post-infectieuse (âge moyen : 51,38±17,78, 7 femmes), 4 patients post-traurnatique (âge moyen : 53,75±9,81, 1 femme), 3 patients sinusite chronique (âge moyen : 46,33±14,50, 1 femme), 8 patients idiopathiques (âge moyen : 63,25±14,18, 3 femmes) et 3 patients avec une étiologie autre (âge moyen : 62,25±16,80, 3 femmes). Il n'y a pas de différence significative entre les différents groupes au niveau de l'âge (p=0,394) et du sexe (X"2 = 8,929, p= 0,063). Afin de pouvoir comparer la population étudiée avec un groupe contrôle, nous avons comparé les résultats de notre cohorte avec une sélection de cas contrôles (appariés pour l'âge et le sexe) de l'étude réalisé par le Docteur Huart au sein du service ORL à Saint-Luc. Nous disposons pour les contrôles des valeurs du test trigéminal. Les Sniffin' Sticks Test, n'ont pas été réalisés chez ces derniers.Pour chacun, nous avons enregistré leurs scores olfactifs, mesurés à !'aide des « Sniffin' Sticks test ». Ces derniers évaluent plusieurs aspects de la fonction olfactive : seuil, discrimination et identification. Ils sont réalisés en unirhinale. Nous avons également évalué leur fonction trigéminale à l'aide d'un test psychophysique analogue aux « Sniffin' Sticks Tests ». Ce « Trigeminal Sticks Test » a été développé, entre autres, par le Dr C. Huart et le Prof. Ph. Rombaux. Ce test reprend les mêmes principes que les « Sniffin' Sticks Tests ». Nous mesurons ainsi le seuil, la discrimination et l'identification. Ces mesures sont faites ici de manière birhinale. Cependant, un volet supplémentaire, la latéralisation, a été ajouté. Conclusion :(1)Le test est donc capable d'évaluer la fonction trigéminale mais en fonction de l'âge uniquement : plus le patient est âgé, plus les scores au test sont bas. Par contre, dans notre population pathologique, le facteur sexe, le statut tabacologique, la présence ou non de parosmies ou phantosmies, n'a pas d'impact sur les résultats alors que dans une population saine, le facteur sexe est important.(2) En considérant les étiologies post-infectieuses, post-traurna et sinusite chronique, les personnes âgées seront moins performantes que les plus jeunes pour les tests olfactifs, ce qui n'est pas le cas chez les patients « idiopathiques ». Quelle que soit l'étiologie, il y aura une atteinte olfactive et une atteinte trigéminale, mais cette dernière ne montre pas de schéma spécifique selon le type d'étiologies.Le test trigéminal est donc intéressant à employer en routine clinique pour déterminer l'importance de l'atteinte de la fonction trigéminale, et donc la capacité ou non de maintenir son rôle de sentinelle des voies respiratoires. En revanche, il n'existe pas de schémas reproductibles pour confirmer le type d'étiologie responsable de l'atteinte. L'asymétrie olfactive n'est pas majorée selon les différents types d'étiologies.

Improvised explosive devices (IEDs) have been one of the leading causes of death and injury among U.S. troops. Those who survive an IED blast or other injuries may be left with a traumatic brain injury (TBI) and attendant or co-occurring psychological symptoms. In response to the need for specialized services for these populations, the U.S. Department of Defense (DoD) established the National Intrepid Center of Excellence (NICoE) in Bethesda, Maryland, in 2010. The NICoE's success in fulfilling its mission is impacted by its relationships with home station providers, patients, and their families. The RAND Corporation was asked to evaluate these relationships and provide recommendations for strengthening the NICoE's efforts to communicate with these groups to improve patients' TBI care. Through surveys, site visits, and interviews with NICoE staff, home station providers, service members who have received care at the NICoE, and the families of these patients, RAND's evaluation examined the interactions between the NICoE and the providers responsible for referring patients and implementing treatment plans. --
