Abstract

The ESP Coordinating Center (ESP CC) is responding to a request from the Center for Compassionate Care Innovation for an evidence brief on the use of transcranial magnetic stimulation (TMS) for the treatment of mental and physical health diagnoses (not including major depressive disorder). Findings from this evidence brief will be used to inform a VHA pilot program to provide access to TMS for Veterans suffering from chronic pain, post-traumatic stress disorder (PTSD), traumatic brain injury (TBI), opioid addiction, or sexual trauma as required by HR 1162, "No Hero Left Untreated Act".

Abstract

Edited by clinicians who were involved with transcranial magnetic stimulation (TMS) from the beginning, Transcranial Magnetic Stimulation: Clinical Applications for Psychiatric Practice offers everything the mental health practitioner needs to know about this innovative and well-established treatment. It is increasingly clear that different combinations of biological, neurobehavioral, and symptomatic factors contribute to the problem of'treatment resistance'in psychiatric disorders. Fortunately, a number of neuromodulation approaches, including TMS, are providing more options for clinicians to combat psychiatric problems. However, guidance about how to identify patients who are good candidates for TMS and how to comanage them during treatment is scarce because instruction on this modality has yet to be integrated into most psychiatry residencies. Abstract

This book is a comprehensive reference on therapeutic repetitive transcranial magnetic stimulation (rTMS) that documents the current status in the field. While introductory chapters cover the neurophysiology of rTMS and present imaging information about its mechanisms of action, the main focus of the book is the clinical applications of rTMS that have been tested to date. These include treatment of paresis, aphasia, and visual neglect in stroke patients, therapy for motor impairment in Parkinson’s disease, and applications for tinnitus and neuropathic pain. Based on the available clinical evidence (RCTs, meta-analyses, and systematic reviews), combined with the personal experience of experts in the field, a clinically oriented best evidence synthesis is provided for each therapeutic application, together with a clear description of rTMS algorithms that generate clinical benefits in the target domain. Abstract

In this work, the physiological effects of time-varying magnetic fields up to 100 kHz have been investigated, namely magnetic stimulation and body warming. Simulation studies were based on numerical calculations on sophisticated cell and body models. In addition, magnetic stimulation thresholds have been determined experimentally.The project was carried out within the scope of the development of Magnetic Particle Imaging, a new imaging technology for medical diagnostics.

Abstract

Despite increased knowledge, and more sophisticated experimental and modelling approaches, fundamental questions remain about how electricity can interact with ongoing brain function in information processing or as a medical intervention. Specifically, what biophysical and network mechanisms allow for weak electric fields to strongly influence neuronal activity and function? How can strong and weak fields induce meaningful changes in CNS function? How do abnormal endogenous electric fields contribute to pathophysiology? Topics included in the review range from the role of field effects in cortical oscillations, transcranial electrical stimulation, deep brain stimulation, modelling of field effects, and the role of field effects in neurological diseases such as epilepsy, hemifacial spasm, trigeminal neuralgia, and multiple sclerosis. The format of each (<1000 word) mini-review will begin by posing a question, problem, or challenge. The author(s) will provide a brief review of the literature and introduce essential concepts. Data regarding the outstanding question is presented, analysed, and issues of contention discussed thoroughly. The author should offer the reader an answer to the posed question if possible, and offer suggestions for future experiments to address outstanding issues.
