Liposuction is the first cosmetic procedure to change beautification surgery from open extensive excision surgery into a more atraumatic closed one. It gave rise to the modern understanding of minimally scarring and minimally invasive surgery and changed the understanding and preferences of both patients and doctors. It also became the most common procedure in cosmetic surgery world-wide, practiced by an increased number of physicians from various specialties. The techniques of fat grafting, closely bound with liposuction, have found widespread application and fat stem cells seem to be changing the future of many areas in medicine. Liposuction is currently the number one aesthetic surgical procedure performed by plastic surgeons in the United States and Americans spend more money on liposuction than on any other aesthetic surgical procedure. Almost 500,000 cases are currently performed yearly by board certified plastic surgeons and it is estimated that at least 20% of the cases are ultrasound assisted. Current ultrasonic liposuction devices are significantly different than the previous generations and require a certain level of technical expertise in order to achieve good outcomes with the technique. Although there are chapters devoted to the technique in general body contouring textbooks, there are no current textbooks that fully cover this important technique. Liposuction began as a contouring procedure but has evolved into the treatment of obese patients, gynecomastia, ptosis, macromastia, and even patients who have complications from heart disease or diabetes. Other disorders such as axillary sweat hypersecretion, lipomas, and angiomas are also potential disorders that may be treated with liposuction. As plastic surgeons, we seek to combine art and science to improve the results we see in clinical practice. Through our artistic sensibilities, we try to understand and obtain aesthetic results. Scientifc analysis provides the data to predict which approaches will be successful and safe. Both art and science connote a high level of skill or mastery. At the present time, our literature is replete with descriptions of specifc pro- dures for body contouring. However, there remains a need for a defnitive reference describing the basic principles to address the complete scope of body contouring including the postbariatric patient and their plastic surgery deformities.
