To determine common haemodynamic outcomes for prosthetic and veinous lower limb infra-inguinal bypasses in order to improve criteria’s characterization for bypasses with an high risk of failure in a more pragmatical way. According to recent results from studies, we think that mean peak systolic velocity (PSV) and end diastolic velocity (EDV) measurement could be such criteria’s.Méthodes :We realized the retrospective study of 116 patients undergoing 134 infra-inguinal bypasses between June 2006 and 2010 , which ones 44% (n=59) was veinous and 56% (n=75) prosthetic. 81% of that bypasses was performed for critical limb ischemia (CU) and 19% for disabling claudication. Hospital, vascular clinic and radiological records. We were able to perform systematically an intra-operative angiography, and for 80% of our bypasses a duplex ultrasound follow-up postoperatively at least two times within the first year and then once a year, from which we took haemodynamic parameters : mean PSV and mean EDV by a minimum of 3 measures on the graft, avoiding stenotic places and anastomosis such as we could not care about characteristic stenotic criteria’s.Results : For all the bypasses, we observed one and two years primary patency of respectively 72 and 67%.0ne and two years secondary patency was respectively 83 and 76%. Our study showed a significant association between patency and mean PSV(Hazard ratio (HR)= 3,6, 95% of confidence limits (Cl) 2,4 - 5,3, p<0.0001). The same association has been made for EDV (Hr= 1.90, 95% of Cl 1.33 - 2.70, p=0.0004). A strong linear relation as been found for PSV (r2=0,92) as for EDV (r2=0,63). Mean graft PSV was associated with patency survival to a similar extent for venous (hazard ratio(HR = 0.42, 95% Cl: 0.23- 0.69) and prosthetic grafts (HR = 0.53, 95% Cl: 0.33-0.86), but not mean EDV. Conclusions : The mean PSV measurement may constitute a more pragmatical outcome than the identification of stenotic lesions to study the patency for prosthetic and veinous infra inguinal bypasses without distinction when using duplex ultrasonography for post intervention follow-up . Déterminer des paramètres hémodynamiques communs tant aux pontages prothétiques que veineux dans le membre inférieur sous-inguinal afin d'obtenir une approche plus pragmatique des critères de caractérisation de pontages à haut risque d'échec. Selon nous, au vu des résultats récents de la littérature, ces paramètres pourraient être les vitesses systoliques maximales (VSM) et télédiastoliques (VTD) moyennes. Méthodes : Nous avons réalisé la revue rétrospective de 116 patients ayant subits la réalisation de 134 pontages infra-inguinaux durant la période de juin 2006 à 2010, dont 44% étaient veineux et 56% prothétiques. 81% de ces pontages ont étés réalisés dans le cadre d'ischémie aigüe du membre inférieur (CLI). Nous avons réalisé un suivi échographique par duplex d'au minimum deux échographies duplex les 6 premiers mois post-opératoires et d'une à un an puis annuellement par la suite pour 80% de ces pontages dont nous avons retranscrit les valeurs de VSM et VTD moyenne par minimum 3 mesures en évitant les lieux de sténose et d'anastomose de manière à pouvoir ne pas tenir compte de leurs critères spécifiques. Résultats : Pour tous les pontages, nous avons observé une perméabilité primaire de un à deux ans respectivement de 72 et 67%. et de respectivement de 83 et 76% pour la perméabilité secondaire. Nous avons montré une association significative entre perméabilité et VSM moyenne(Hazard ratio (HR)= 3,6, 95% d'intervalle de confiance (Cl) 2,4 - 5,3, p<0.0001) La même association a été faite pour la VTD moyenne(HR= 1.90, 95% de Cl 1.33 - 2.70, p=0.0004). Une puissante relation linéaire entre taux d'occlusion et quartiles a été retrouvée pour la VSM (r2=0,92) comme la VTD (r2=0,63). La VSM moyenne a été associée a une survie de perméabilité de façon similaire pour les pontages veineux (HR = 4,2 95% de Cl 2,3 - 6,9) et prothétiques (HR = 5,3, 95% Cl: 3,3 - 8,6) mais pas la VTD. Conclusions : Les mesures de la VSM moyenne pourraient, de manière indistincte pour les pontages veineux ou prothétiques du membre inférieur sous-inguinal, constituer un paramètre plus pragmatique que l'identification de lésions sténotiques de l'étude de leurs perméabilités par échographie lors du suivi post-opératoire.
