Abstract

Le sujet a traité du manque de rentabilité des analyses d'immunophénotypage réalisées en cytométrie en flux au laboratoire d'hématologie du CHU Saint Pierre. L'objectif de ce mémoire est d'une part, de réaliser la description de la situation actuelle de la cytométrie via une analyse partielle des processus et des coûts engendrés par cette activité. Et d'autre part, de proposer des pistes d'amélioration de la gestion technique de la cytométrie. Pour atteindre cet objectif, une analyse de coûts partielle a été menée aux laboratoires d'hématologie du CHU Saint Pierre et l'hôpital Erasme. Dans un deuxième temps, ce mémoire a comparé les deux processus d'analyses d'immunophénotypage suivies dans les deux laboratoires hospitaliers visités. Ces démarches ont eu comme objectif, l'ambition d'améliorer l'efficience de l'activité de la cytométrie en flux du CHU Saint-Pierre. Méthodes : Les coûts des réactifs des 7 types d'analyses d'immunophénotypage, usuellement effectuées au laboratoire pour des patients ambulatoires en diagnostic, ont été décortiqués en coûts par test, par préparation de cocktail et par valorisation. Ces données furent comparées avec celle obtenues au laboratoire d'hématologie de l'hôpital Erasme. Les deux processus étudiés ont été décrits et sectionnés en 8 étapes. Les différences de pratiques y ont été situées. Résultats : Au CHU Saint Pierre, les résultats ont démontrés une variation de coûts entre les analyses effectuées en cytomètre SC et en cytomètre !OC. Les coûts par test d'imrnunophénotypage sont inférieurs à l'hôpital Erasme par rapport au CHU Saint Pierre. De plus, il existe des différences de pratiques de cytométrie en flux entre les deux laboratoires. Ce mémoire a permis l'élaboration de 5 pistes d'amélioration applicables au secteur de cytométrie du CHU Saint Pierre.

Although instrumentation and laboratory techniques for flow cytometry (FCM) immunophenotyping of hematopoietic malignancies are well documented, there is relatively little information on how best to perform data analysis, a critical step in FCM testing. In Flow Cytometry in Hematopathology: A Visual Approach to Data Analysis and Interpretation, three physicians highly experienced in laboratory hematopathology and FCM offer a unique systematic approach to FCM data analysis and interpretation based on the visual inspection of dual parameter FCM graphics. This step-by-step approach to optimal FCM data analysis is demonstrated by means of numerous FCM graphics derived from actual well-documented clinical cases. Abstract

Since the end of World War II, biology and medicine have merged in remarkably productive ways. In this book Peter Keating and Alberto Cambrosio analyze the transformation of medicine into biomedicine and its consequences, ranging from the recasting of hospital architecture to the redefinition of the human body, disease, and therapeutic practices. To describe this new alignment between the normal and the pathological, the authors introduce the notion of the biomedical platform. Defined as a specific configuration of instruments, individuals, and programs, biomedical platforms generate routines, entities, and activities, held together by standard reagents and protocols. Biological entities such as cell surface markers, oncogenes, and DNA profiles now exist as both normal biological components of the organism and as pathological signs--that is, as biomedical substances. The notion of a biomedical platform allows researchers interested in the development of contemporary medicine to describe events and processes overlooked by other approaches.The authors focus on a specific biomedical platform known as immunophenotyping. They describe its emergence as an experimental system with roots in biology (immunology) and pathology (oncology). They recount how this experimental system was transformed into a biomedical platform initially for the diagnosis of leukemia and subsequently for other diseases such as AIDS. Through this case study, they show that a biomedical platform is the bench upon which conventions concerning the biological or normal are connected with conventions concerning the medical or pathological. They observe that new platforms are often aligned with existing ones and integrated into an expanding set of clinical-biological strategies.
