The role of hyperprolactinaemia in male infertility has yet to be discovered. Animal studies indicate that prolactin has a direct effect on spermatogenesis and steroidogenesis. Research on this tapie has led to inconsistent conclusions ; hyperprolactinaemia may be the cause of disturbances in semen analysis and in endocrine tests.Objective : This retrospective study aimed at finding whether hyperprolactinemia has an impact on gonadal function and on fertility in men, through the study of semen parameters and pregnancy rates in infertile couples treated by medically assisted procreation.Methods : Forty five patients consulting for fertility issues in Cliniques Universitaires de Saint Luc between 2008 and 2013 and whom have been diagnosed with hyperprolactinemia were selected. Each of them have been matched according to age and year of consultation to another patient with fertility issues but with a normal prolactinemia . Fertility is evaluated by, semen analysis and the pregnancy rate.Results : LH levels are significantly higher in the study group as opposed to the control group (P=0,026). We have observed a statistically significant correlation between PRL and FSH levels (P = 0,03). There was no statistically significant difference in the semen analysis of the study and control group. In the study group, the pregnancy rate after the first in vitro fertilization was significantly higher than in the control group.Conclusion : This study has not revealed an impact of hyperprolactinaemia on semen parameters in the infertile population. However, we observed a higher pregnancy rate in the study group following the first in vitro fertilization . Our results have yet to be confirmed by other studies with a larger cohort of patients or a prospective design. Le rôle de l'hyperprolactinémie dans l'infertilité masculine est encore méconnu. Chez l'animal, la prolactine a un effect direct sur la spermatogenèse ainsi que sur la stéroïdogenèse. Les études menées chez l'homme sont contradictoires ; une hyperprolactinémie pourrait engendrer une altération du spermogramme ainsi que des perturbations de l'axe hypothalamo-hypophyso-gonadique.Objectifs : L'objectif de cette étude rétrospective est d'étudier s'il existe un effet de l'hyperprolactinémie sur la fonction gonadique de l'homme ainsi que sa fertilité, évaluée par l'effet sur les paramètres spermatiques et le taux de grossesse des couples infertiles traités par procréation médicalement assistée.Méthodes : Quarante-cinq patients suivis pour stérilité aux Cliniques Universitaires de Saint Luc entre 2008 et 2013 et chez qui une hyperprolactinémie a été mise en évidence au cours du bilan, ont été sélectionnés pour constituer le groupe étude. Chaque patient a été matché pour l'âge et l'année de consultation avec un autre patient suivi pour stérilité mais avec un taux de prolactine normal. La fertilité est évaluée en fonètion de la qualité spermatique et du taux de grossesse obtenu au cours de l'étude.Résultats : Nous trouvons un taux de LH significativement supérieur dans le groupe étude par rapport au groupe contrôle (P=0,026), ainsi qu'une corrélation positive statistiquement significative entre le taux de PRL et FSH (P=0,03).Nous n'avons pas trouvé de différence statistiquement significative des paramètres spermatiques entre le groupe étude et le groupe contrôle. Dans le groupe étude, nous retrouvons un taux de grossesse significativement supérieur au groupe contrôle au terme de la première tentative de FIV (P= 0,05).Conclusion : Nous n'avons pas mis en évidence d'impact de l'hyperprolactinémie sur les paramètres spermatiques de la population infertile. Toutefois, nous observons un taux de grossesse supérieur dans la population de patients hyperprolactinémiques au terme de la première tentative FIV. Les résultats sont à confirmer par d'autres études avec une cohorte supérieure ou un design prospectif.
