Trois cent cinquante millions de porteurs chroniques dans le monde, environ un million de décès chaque année, un vaccin efficace mais décrié en France, des traitements de plus en plus performants mais inaccessibles dans les régions les plus touchées par l’endémie. Près d’un demi-siècle après la découverte du virus de l'hépatite B, les défis posés par l'infection restent considérables. L'estimation récente de l'Institut de veille sanitaire a montré que la prévalence de l'infection par le virus de l'hépatite B était voisine de celle de l'infection par le virus de l'hépatite C en France. Les populations migrantes et les couches les plus défavorisées de la société sont les plus touchées. Le développement récent de molécules antivirales puissantes possédant une barrière génétique élevée contre la résistance permet en théorie de contrôler la très grande majorité des hépatites chroniques B. Cependant, l'obligation de traiter longtemps, sans doute à vie, une infection a priori incurable pose des problèmes difficiles d'indication thérapeutique, d'observance et de résistance. Aux campagnes de vaccination massive des nouveau-nés, enfants et adolescents dans la plupart des pays d'Asie, d'Europe et d'Afrique, la France oppose un décalage qui devrait être rapidement corrigé. La publication d'un ouvrage de référence sur l'hépatite B, après celui sur l'hépatite C, s'imposait donc. Les meilleurs spécialistes français se sont réunis pour faire le point sur les connaissances les plus récentes, sous l'oeil bienveillant de Harvey Alter, co-découvreur du virus de l'hépatite B avec Baruch Blumberg, prix Nobel de Médecine, alors que l'European Association for the Study of the Liver (EASL) publiait ses première recommandations de pratiques cliniques sur la prise en charge de l'hépatite chronique B et que notre ministre lançait, pour les années 2009-2012, un nouveau plan de lutte contre les hépatites virales. La publication de cet ouvrage arrive, nous semble-t-il, bien à propos.
