Abstract

This study presents a broad overview of health-system reforms in OECD countries over the past several decades. Reforms are assessed according to their impact on the following policy goals: ensuring access to needed health-care services; improving the quality of health care and its outcomes; allocating an "appropriate" level of pubic sector and economy-wide resources to health care (macroeconomic efficiency); and ensuring that services are provided in a cost-efficient and cost-effective manner (microeconomic efficiency).While nearly all OECD countries have achieved universal coverage of health-care risks, initiatives to address persistent disparities in access are now being undertaken in a number of countries. In light of new evidence of serious problems with health-care quality, many countries have recently introduced reforms intended to improve this, but it is too soon to generalise as to the relative effects of alternative approaches. Abstract

Dans cette nouvelle édition actualisée, Françoise Molénat rend compte d'une démarche éthique novatrice, initiée en fin des années 1970, actuellement en pleine dynamique. Elle pose les bases éthiques de tout travail de prévention précoce mené par les équipes de périnatalité auprès des familles en souffrance. La prévention de la maltraitance, des troubles du développement, des dépressions parentales est à portée de main. Changer les habitudes exige un effort collectif pour que les regards se croisent, que les cultures se métissent et que la position de « prévenance » s'impose avant la survenue des troubles en santé mentale. Pour cela, sont requises la rigueur interdisciplinaire et surtout une exigence éthique quant au respect de la place de sujet de chaque interlocuteur concerné, sans laquelle des parents blessés dans leur image d'eux-mêmes risquent de fuir toute proposition d'accompagnement. Des témoignages émaillent cette réflexion qui s'appuie sur la subjectivité des parents, mais aussi sur celle des intervenants proches d'une famille en construction. Les questions éthiques inhérentes à l'avancée dans la part intime des familles sont le fil directeur de l'ouvrage.

Such teams, when structured in a cohesive fashion, can function more effectively and efficiently than the sum of their parts. The benefits of teamwork extend from the delivery of care to a single patient to the overall structure and function of entire care delivery systems. Recognizing the value of collaborative approaches for improving all aspects of healthcare delivery and having champions, leaders, structure, function, goals, and accountability are paramount to success, regardless of how defined. Another important pillar of teamwork is excellent communication with clearly defined information flows and cross-verification mechanisms. This book outlines how to work together for shared goals in a complex, diverse, and constantly evolving health care system.

Abstract

This report discusses the remarks delivered by World Bank Group President David Malpass at the G20 Finance Ministers and Central Bank Governors meeting held during the 2022 annual meetings. He discusses: the world needs to be much better prepared to respond to pandemics, catastrophes, and health emergencies. Investing in prevention and preparedness now will save lives and, over the medium term, will save resources. Since the COVID-19 pandemic is not yet behind us, let me first say a few words about our efforts to support countries through this pandemic. I'll then turn to the topic of preparedness and financing for future health crises. The World Bank Group's 157 billion dollars response to COVID-19 was unprecedented. It was the fastest and largest growth in our history. This helped countries evaluate their health capacity gaps, finance their health systems, secure vaccines, and turn those vaccines into actual vaccinations in the poorest countries. As of last week, our financing has helped 67 countries purchase over half a billion doses. Doses closely connected. The doses are closely connected to our financing of country deployment programs and monitoring, through workforce training, public awareness campaigns, logistics, cold chain capacity, syringes, and testing kits. Preparedness and building for the future are core to the World Bank's mission through comprehensive health projects. We are working to strengthen health systems in over 100 countries with an active portfolio totaling 30 billion dollars.

Abstract

Rheumatoid arthritis (RA) is a chronic rheumatic, autoimmune disease that causes inflammation of the joints and connective tissues. International research estimates that RA affects between 0.5% and 1% of the adult population. Although there have recently been large advances in pharmacological treatments, many patients still have a need for follow-up and rehabilitation. To best organize future rehabilitation service for patients with RA, there is a need for evaluating and comparing the effect of a specialised, multidisciplinary treatment with a non-multidisciplinary approach. The aim of this systematic review is to compare the effect of a multidisciplinary, team-based rehabilitation program, including education, specialised for patients with RA with treatment/training without a multidisciplinary team, or with no special follow-up. We included eleven studies. Six studies were about patient education as sole intervention, and five studies were about multidisciplinary team-based rehabilitation. The main findings from these studies were:1. Patient education delivered as sole intervention, gives an improvement in symptoms, self-efficacy and knowledge about RA at one year follow-up in favour of the intervention group. For other outcomes, patient education showed small or no effects.2. Multidisciplinary team-based rehabilitation showed, for HAQ physical function, an improved score in favour of the control group at one year follow-up. At two years follow-up there still was a tendency towards better scores in the control group, but the difference was no longer significant. For other outcomes, multidisciplinary, team-based rehabilitation showed small or no effect. The documentation about the effect of multidisciplinary team-based rehabilitation is insufficient, and the conclusions are based on findings from small studies with unclear or high risk of bias. It is important to emphasize that no evidence of effect is not the same as evidence of no effect.
