Because of its high impact on both animal and human health, avian influenza has become a matter of increasing public concern and growing scientific interest within the last decade. This volume gives an overview of the most important results of these research efforts and provides information about the ecology and epidemiology of avian influenza with particular emphasis on recent H5N1 outbreaks in China, Siberia and Europe. Several articles deal with new vaccination strategies, the use of antivirals and other control measures to combat outbreaks of avian influenza. Further chapters illustrate that molecular biology, culminating in the generation of influenza viruses by recombinant DNA technology, was instrumental in unravelling the roles of the viral hemagglutinin and polymerase as well as cellular signalling pathways and innate immunity in pathogenesis and interspecies transmission. Finally, the threat of a pandemic originating from avian influenza viruses is illustrated by the example of the Spanish influenza of 1918. La pandémie grippale A(H1N1) a surpris, en avril dernier, ceux qui s'y préparaient pourtant depuis des années. Aucun dispositif n'a permis de limiter l'extension du phénomène désormais planétaire. Il est vraiment urgent d'anticiper son impact si, dans les mois qui viennent, le virus déjouait les stratégies et générait un désastre sanitaire peu contrôlable. Les plans nationaux conçus pour assurer l'accès aux traitements et la continuité de la vie publique énoncent des dispositifs dont on ignore en pratique l'efficacité. Face à une menace imprévisible et dont on ne sait quelle en serait l'intensité, peut-on s'en remettre en toute confiance à des procédures dont la mise en œuvre dépendra pourtant directement de la motivation des personnes ? Aujourd'hui, qui peut affirmer sérieusement que notre société est prête à assumer des défis dont, en fait, elle ne sait rien de précis ? Qui est assuré que les solidarités seront plus fortes que l'individualisme du « sauve-qui-peut » ? Il est encore temps de créer les conditions d'une concertation publique qui permette de sensibiliser le corps social afin de le mobiliser. À eux seuls, les stocks d'antiviraux, de masques et les vaccins ne constituent qu'un aspect partiel des réponses exigées face à une menace pandémique. Confrontée à des choix difficiles, la société préservera-t-elle les valeurs de démocratie ou versera-t-elle dans l'état d'exception ? Selon quels critères les décisions seront-elles prises ? Quelles instances en assureront l'arbitrage lorsqu'elles auront pour conséquence la vie ou la mort de personnes ? Notre pays saura-t-il, en situation extrême, honorer la tradition des droits de l'homme et témoigner sa sollicitude à l'égard des plus vulnérables, notamment auprès des populations démunies de toute ressource thérapeutique ? Même si la menace pandémique s'estompait dans les prochains mois, il est important d'avoir le courage d'aborder dignement les questions de fond. À vouloir les éviter ou les évincer, l'irresponsabilité et l'indifférence gagnent sur la lucidité et l'esprit d'initiative. Il s'agit là d'une première défaite qui rend dès lors plus incertain encore l'engagement qui s'impose pourtant à tous. Ce livre est un acte politique : il se propose de contribuer dans l'urgence à l'indispensable débat public organisé dans les prochaines semaines. Associant les meilleures compétences, il présente des pistes de réflexions et des propositions nécessaires en temps de pandémie.
