Introduction: The cervico-maxillo-facial defects require a functional and aesthetic reconstruction of the missing area. The vascularized osseous free graft form the fibula is the most adapted option to substitute the bone defect. The early clinical examination is essential after surgery during the two first week. The clinical flap evaluation is limited to the skin portion’s assessment when one is associated. However a mismatch may exist between the viability of the bony and skin portions such as a clinical skin evaluation attesting a normal perfusion doesn’t permit to conclude to the bone viability. Thereof the bone scintigraphy is a good alternative to evaluate the bone graft. The aim of this study is to assess the contribution of the bone scintigraphy for the postsurgical viability assessment of the fibular microvascular free flap. Materials and Methods: Data from 70 fibular mandibular reconstruction s between 1992 and 2010 in the Saint-Luc University Clinics have been collected. Ten of 70 interventions have been excluded for a lack of data regarding the bone scintigraphy results. A 3 phases scintigraphy with 99mTc-MDP (13 interventions) and 99mTc-HDP (47 interventions) has been systematically done between 2 to 14 days after surgery to access the viability of the bone graft. Outcomes: Fifty-four of 60 fibular graft reconstructions succeeded (90%). Six flaps have benne lost during the post-surgical period. The scintigraphy showed a viability for 48 of 54 flaps that have survived (specificity: 88.9%) The scintigraphy suggested a non-viability for 3 of 6 grafts that have been lost (sensibility: 50%)? The negative predictive value is 94.1% and the positive predictive value (the detection of flaps that have been effectively lost) is 33.3%. Conclusion: The three-phase bone scintigraphy performed within the 14 days following the mandibular reconstruction by fibular microvascular free flap is reliable to confirm the bone graft’s viability. However, the positive predictive value of this study isn’t coherent with the literature results. Some explanations for this variance have benne discusses. A new analysis of the bone scintigraphy’s pictures according to the standards that have been described in the literature could explain with more prevision the probability of flap’s survival. Introduction: Les déficits de substances cervico-maxillo-faciales nécessitent une reconstruction Assurant la fonctionnalité et l’esthétique de la région concernée. Le lambeau osseux fibulaire microvascularisé est une indication de choix afin de combler la perte osseuse. L’évaluation de la viabilité précoce de celui-ci est essentielle durant les deux semaines suivant l’intervention chirurgicale. L’étude clinique de la vascularisation du lambeau osseux est limitée à l’appréciation de la perfusion de la palette cutanée lorsqu’elle est associée au greffon. Le manque de concordance entre la viabilité des portions cutanée et osseuse du lambeau ne permet cependant pas de déterminer avec certitude la vascularisation de cette dernière. Dès lors, la scintigraphie osseuse est un examen de choix afin d’évaluer la viabilité du greffon osseux. La finalité de cette étude est d’étudier l’apport de la scintigraphie osseuse lors de la détermination post-opératoire de la viabilité du lambeau osseux fibulaire microvascularisé. Matériel et méthode : Nous avons recueilli les données de 70 reconstructions mandibulaires par lambeau fibulaire osseux microvascularisé réalisées aux Cliniques Universitaires Saint-Luc entre 1992 et 2010. Dix interventions sur 70 ont été exclues par manque de données concernant les résultats de la scintigraphie osseuse. Une scintigraphie osseuse en 3 phases au 99mTc-MDP (13 interventions) et au 99mTc-HDP (47 interventions) a été systématiquement réalisées entre 2 à 14 jours durant la période post-opératoire afin d’évaluer la viabilité ou non de la portion osseuse du lambeau. Résultats : Cinquante-quatre des 60 interventions de reconstruction par lambeau osseux fibulaire microvascularisé ont été un succès (90%). Six lambeaux osseux ont été perdus durant la période post-opératoire. La scintigraphie démontrait une viabilité pour 48 des 54 lambeaux ayant survécu (spécificité 88.9%). La scintigraphie suggérait une absence de viabilité pour 3 des lambeaux perdus (sensibilité : 50%). La valeur prédictive négative est de 94.1M et la valeur prédictive positive (détection des lambeaux effectivement non-viables) est de 33.3%. Conclusion : La scintigraphie osseuse en 3 phases réalisée endéans les 14 jours suivant l’intervention de reconstruction mandibulaire par lambeau osseux fibulaire microvascularisé est statistiquement fiable pour confirmer la viabilité de la portion osseuse du lambeau. Dans le cadre de cette étude rétrospective, les résultats sont inférieurs à la littérature. Les biais et améliorations possible sont discutés.
