This book offers a highly informative guide to treating unexplained infertility and recurrent miscarriage (RM). In particular, it provides detailed treatment strategies for infertility or RM derived from uterine circumstance such as chronic endometritis and perturbation of endometrial decidualization, as well as maternal immunological rejection of an embryo as semi-allograft. Unexplained infertility refers to those types that cannot be detected by the general screening test. The causes are sometimes detected in the course of treatment with assisted reproductive technology including IVF. However, some unexplained infertility is intractable even after intracytoplasmic sperm injection or repeated implantation of morphologically suitable embryos. Patients with unexplained RM also have a high likelihood of undetectable risk factors of miscarriage. As a result, gynecologists often repeatedly provide these couples with general treatments for infertility and miscarriage or even discontinue treatment because they cannot detect the reason, which places serious financial, physical and mental burdens on the couples affected. This book offers gynecologists essential insights into the pathological condition of unexplained infertility and RM, equipping them to identify it, explain it to patients, and consider further examinations and more aggressive fertility treatments.

This book brings together the most current research and the latest clinical approaches to the management of diminished ovarian reserve (DOR), one of the largest segments of the IVF patient population, both in the advanced reproductive age group as well as poor responders. Opening with a review of the definition and scope of the problem, as well as the current understanding of the natural history of DOR, subsequent chapters in part I outline dietary, hormonal, traditional supplements and conventional methods used to stimulate ovaries and improve ART outcomes. The main segment of chapters, comprising part II, present minimal and mild stimulation protocols and alternatives, frozen embryo transfer preparation, trigger agents and post-trigger testing, embryo culture and endometrial considerations, and a review of clinical outcomes. Part III discusses the utilization of contemporary technologies in the treatment of DOR, including fresh vs. frozen embryo transfer, cryopreservation and comprehensive chromosomal analysis. Future prospects are presented in part IV, such as the artificial oocyte and ovary development, early-age oocyte freezing, ovarian cortical tissue freezing and activation of the ovarian cortex. Utilizing the latest evidence and authored by an international array of thought leaders, Diminished Ovarian Reserve and Assisted Reproductive Technologies is an excellent resource for reproductive medicine and REI specialists, IVF lab professionals, and students and residents in these areas.

This book examines risk in assisted conception, covering a wide range of topics and areas. The authors discuss the ‘Risk’ specifically for situations where patients or professionals are at risk of an adverse outcome in relation to fertility treatment. Reducing Risk in Fertility Treatment encompasses clinical complications, risks inherent in laboratory practice, reputational and business risks to clinics, and ethical risks to society as a whole. Reducing Risk in Fertility Treatment is an essential resource for all clinical staff to consult in reducing the risk associated with their work. It provides practical resources such as ‘how to’ guides, but also touches on theoretical discussions. It is primarily directed to practitioners in the area of fertility treatment, clinicians, embryologists, nurses, counsellors and administrators, but is also relevant to policy-makers, commissioners of health services, researchers in this field and biomedical ethicists.
