Le médecin généraliste est régulièrement confronté à des éruptions cutanées ; les étiologies possibles sont multiples et parfois intriquées. Nous allons nous limiter au diagnostic étiologique des érythèmes, macules et papules. Pour cela, apprendre à décrire la lésion est d’une grand aide.
Le travail effectué a été de reprendre les principales pathologies cutanées en Médecine Générale et de mettre en évidence leurs caractéristiques lésionnelles, anamnestiques.
Le type de travail choisi est une présentation orale au sein d’un dodécogroupe. Celle-ci est réalisée sur base d’un feuillet d’images et de transparents élaborés à partir du texte qui suit

On peut diviser les rashes selon leur type de lésion cutanée produite. Tout d’abord l’éruption scarlatiniforme, se décrivant comme un érythème micropapuleux accompagnée d’une desquamation des extrémités, se rencontre aussi bien dans la scarlatine, la maladie de Kawasaki et lors de la prise de médicaments :
- La scarlatine due au streptocoque β hémolytique de type A se traite par un antibiotique et l’enfant doit être écarté de l’école.
- La maladie de Kawasaki qui provoque de graves complications conduit à l’hospitalisation de l’enfant.
On retrouve ensuite l’érythème maculopapuleux qui se répartir en deux entités selon la couleur de la lésion : le rash morbilliforme (macules rouges, plutôt papuleux) et le rash roséoliforme (macules roses, plus discret). Le premier type d’éruption comprend le rougeole, l’érythème infectieux et la mononucléose. Le second, la roséole, le CMV et la rubéole. Actuellement rare grâce à la vaccination, la rougeole provoque de graves complications. L’érythème infectieux, pathologie fréquente et bénigne des enfants de 4 à 10 ans, est facile à reconnaître avec son « syndrome des joues giflées ». Fréquente également mais étant plus légère, la mononucléose se remarque lorsqu’une pénicilline est prescrite. L’exanthème subit ou la roséole, atteignant les enfants entre 6 mois et 2 ans, n’est pas rare en médecine générale. Etant plus diversifiée, l’éruption causée par le CMV n’est que très rarement diagnostiquée et la vaccination a pratiquement éradiqué la rubéole dont les répercussions sont sévères pour le fœtus.
Troisièmement, vient l’érythème vésiculeux avec la varicelle, le zona, l’herpès simplex, le syndrome main-pied-bouche, la gale et la pédiculose. Diagnostic fait pas les parents, la varicelle reste relativement bénigne chez l’enfant immunocompétent. La vaccination anti-varicelle sera probablement rentrée dans les mœurs d’ici quelques années. Plus fréquemment rencontré chez l’adulte, le zona ne cause pas de douleurs post-zonateuses chez l’enfant. L’infection à herpès HSV1 touche la plupart des enfants avant l’âge de 5 ans. Elle se manifeste par la gingivostomatie et se réactive plus tard sous la forme du bouton de fièvre
