Abstract

En raison de son importance, l'alimentation a une place à part dans le paysage développemental du bébé. A priori solidement ancrée dans la physiologie, elle s'avère plus fragile qu'il n'y paraît. Les recherches épidémiologiques récentes montrent que 25 % à 35 % des bébés en clinique pédiatrique ordinaire sont sujets à des difficultés d'alimentation restrictive. Pour 2% des bébés et jeunes enfants, ces difficultés peuvent devenir plus importantes et se transformer en grave trouble du comportement alimentaire mettant leur vie en danger. Les troubles précoces du comportement alimentaire sont complexes et hétérogènes. Les protagonistes sont nombreux, les causalités multiples et les intrications somatopsychiques importantes. À partir de l'histoire de six bébés qui souffrent d'un trouble du comportement alimentaire différent, l'auteur s'attache à dégager des pistes étiologiques mais aussi thérapeutiques. Le référentiel psychanalytique de l'auteur est enrichi par l'observation du bébé, par la recherche pédiatrique et neuroscientifique ainsi que par l'approche pluridisciplinaire de son expérience au sein de l'hôpital pédiatrique de Toulouse.

Abstract

Les troubles des conduites alimentaires constituent un motif fréquent de consultation, essentiellement chez l'adolescent mais aussi chez l'enfant. Par leur aspect parfois extrême, par les risques évolutifs qu'ils présentent, ils mobilisent les soignants et nécessitent souvent des interventions en urgence. Qu'ils se présentent sous la forme de l'anorexie ou de la boulimie, voire de leur alternance, les symptômes n'ouvrent pas sur la compréhension du fonctionnement psychique qui les sous-tend. Le bilan psychologique, éclairé par la référence à la théorie psychanalytique, contribue à la connaissance, non pas du trouble, mais du sujet qui le présente. Il offre à ce titre une aide à la prise en charge pour l'équipe soignante et une ouverture à la compréhension pour la famille.
