La langue Doc(k)s a révolutionné la poésie du XXe siècle. Revue internationalement reconnue, crée par Julien Blaine en 1973 puis poursuivie par Akenaton, Doc(k)s a fait plusieurs fois le tour du monde, donnant lettre, corps, image aux poésies expérimentales du XXe siècle. Desaguar em cinema, por meio de um ajuntamento de vozes diversas de pesquisadoras(es), cineastas e ativistas, em abordagens multidisciplinares, tenta dar vazão ao fértil movimento do documentário brasileiro contemporâneo que nutriu e foi nutrido pelo festival pioneiro no gênero Norte-Nordeste no Brasil. Trata-se de um festival – e de um livro – dedicado ao cinema impregnado pela vida.

Abstract

The main objectives of this thesis were to characterize the pattern of physical activity in daily life in patients with Chronic Obstructive Pulmonary Disease (COPD) and to investigate the impact of acute exacerbations and pulmonary rehabilitation onthese patients’ physical activity level. Results showed that most COPD patients spend significantly less time walking and standing and more time sitting and lying in daily life when compared to age-matched sedentary healthy subjects. In addition, itwas shown that COPD patients not only walk less time per day but also walk 25% more slowly. These results suggested that even the most active COPD patients probably have an activity level insufficient to result in fitness maintenance. We alsoshowed a marked inactivity of COPD patients during and after a severe acute exacerbation. This inactivity was related to a decrease in peripheral muscle force and to a higher chance of hospital readmission in the following year. The relation of inactivity with these potential clinical consequences demonstrates the need for strategies to enhance physical activity during and after acute exacerbations. Results from this thesis also showed that changes in physical activities in daily life after 3 months of pulmonary rehabilitation in stable patients with COPD were limited to a borderline significant improvement in movement intensity during walking in daily life. Improvement in time spent walking in daily life was only obtained after 6months of pulmonary rehabilitation, together with further improvement in movement intensity. This lack of improvements in walking time in daily life after 3 months was not due to lack of improvement in functional exercise capacity and functional status since these outcomes improved over this period of time. Therefore, it could be reasoned that COPD patients require rehabilitation programs that last longer than 3 months in order to translate the improvements obtained with the program into amore active lifestyle. Furthermore, it was shown that the occurrence of a severe acute exacerbation requiring hospitalization did not jeopardize improvements obtained with the 6-month rehabilitation program. In summary, this thesis indicates that interventions aiming to implement a more active lifestyle are imperative concerning COPD patients’ management. Chronic Obstructive Pulmonary Disease (or COPD, also known as Chronic Bronchitis and/or Emphysema) is a disorder mainly caused by smoking and characterized by airflow obstruction. COPD is a major cause of morbidity and mortality. It is currently ranked as the fourth leading cause of death in the world, and is expected to be the third by 2020. Mortality rates due to COPD are increasing, in contrast to all other severe chronic diseases. The burden of dealing with the disease-related morbidity is enormous. For all these reasons, management of patients with COPD is nowadays an issue of great concern. COPD is typically characterized by breathlessness, although fatigue, muscle weakness, physical deconditioning and reduced quality of life are also common features. As the disease progresses, it is common that patients become severely restricted in terms of physical activity in daily life. However, detailed information concerning physical activity in daily life in patients with COPD is not available in the scientific literature. This thesis was devoted to characterize the pattern of physical activity in daily life in patients with COPD and to investigate the impact of acute exacerbations and pulmonary rehabilitation on these patients’ physical activity level. Results showed that most COPD patients are markedly inactive in daily life, especially when compared to healthy sedentary subjects of the same age. In addition, it was shown that COPD patients not only walk less time per day but also walk 25% more slowly. These results suggested that even the most active COPD patients probably have an activity level insufficient to maintain their fitness. During hospitalization for acute exacerbation, physical activity of COPD patients is even more markedly reduced. This further reduction in physical activity during exacerbations was related to a decrease in leg muscle strength and to a higher chance of hospital readmission in the following year. These important implications of inactivity demonstrate the need for strategies to increase physical activity during and after acute exacerbations. Results from this thesis also showed that a 3-month rehabilitation program resulted in important improvements in quality of life,muscle force and exercise capacity of stable COPD patients. However, changes in physical activity in daily life after these 3 months of rehabilitation were limited to an increase in the average walking speed. Improvement in time spent walking in daily life was only obtained after 6 months of rehabilitation, together with further improvement in walking speed. Therefore, it could be reasoned that COPD patients require rehabilitation programs that last longer than 3 months in order to translate the improvements obtained with the program into a more active lifestyle. In summary, this thesis indicates that interventions aiming to implement a more active lifestyle are imperative concerning COPD patients’ management.
