Biophosphanates are agents that belong to the current pharmacological arsenal used for the prevention and the treatment of the various types of osteoporosis. Their mechanism of action is complex: their main effect, however, consists in the inhibition of osteoclastic bone resorption, consequently providing a decrease in the risk of bone fractures. Their use, whether in the form of per os pills or intravenous injections, has been reported as potential cause for severe side-effects such as atypical femoral fractures, osteonecrosis of the jaw, atrial fibrillation, and esophageal cancer. Some properties of the biphosphonates drugs can explain the occurrence of such events (i.e. their negative effects in bone renewal and the formation of collagen, their toxicity on oral and hut mucosa, their anti-angiogenic effect, and their pro-inflammatory action). However, follow-up studies are still necessary to estimate the situation on a more definitive way. In order to avoid an increase in the number of patients presenting with side-effects, health care professionals, both physicians and pharmacists, need to play a pro-active role in educating their patients about the potential risks and precautions, and provide the appropriate follow-up Les biphosphonates font partie de l’arsenal thérapeutique de la prévention et du traitement des différents types d’ostéoporose. Certaines propriétés des biphosphonates permettraient d’expliquer le survenue de ces événements : leur effet négatif sur le remodelage osseux et sur la formation de collagène, leur toxicité pour les muqueuses, leur pouvoir anti-angiogénique et leur action pro-inflammatoire. Néanmoins, des études complémentaires sont encore nécessaires pour évaluer la situation de façon définitive.
