Analisa as questões relativas ao agente comunitário de saúde, decorrentes da grande disseminação dos Programas de Saúde da Família. Estudando o agente comunitário de saúde que atua no Projeto Qualis/PSF, no município de São Paulo, este livro examina a construção de identidades profissionais e caracteriza o perfil das áreas metropolitanas. Por meio de uma reflexão sobre a identidade dos profissionais dos programas de saúde, os agentes comunitários, e sobre suas dificuldades para viabilizar uma política de reorientação do modelo de assistência à saúde, este livro demonstra sua importância como discussão essencial para sustentar e continuar os Programas de Saúde da Família, buscando recursos disponíveis nas próprias comunidades.

Abstract

With nearly half of the world's population living in a rural or remote area, meeting the health needs of rural populations, where over 80% of the world's extremely poor live, is imperative in achieving universal health coverage. Leaving no one behind means ensuring that health workers are available in rural and remote areas. Health, social and economic inequities remain cross-cutting challenges for rural populations. Rural populations tend to be poorer, have worse health outcomes, and experience higher rates of unemployment, underemployment and informal employment. It is estimated that about 51-67% of rural populations are without adequate access to essential health services, translating to about 2 billion people being left behind. In some countries, rural populations have access to numbers of health workers that are 10 times less than the numbers available to urban populations. The deficiency in numbers and mix of trained motivated health workers to provide the needed health services is a critical health system issue. This inequitable access to health workers and health services impacts health outcomes and increases socioeconomic disadvantages. Higher under-5, maternal and preventable mortality rates, increased morbidity, decreased life expectancy, and more costs to access distant care are seen across rural areas.

"Insufficient attention has been paid to understanding what determines the performance of health workers and how they make labor market choices. This paper reports on findings from focus group discussions with both health workers and users of health services in Ethiopia, a country with some of the poorest health outcomes in the world. It describes performance problems identified by both health, users and health workers participating in the focus group discussions, including absenteeism and shirking, pilfering drugs and materials, informal health care provision and illicit charging, and corruption. The second part of the paper presents four structural reasons why these problems arise: (1) the ongoing transition from a health sector dominated by the public sector, toward a more mixed model; (2) the failure of government policies to keep pace with the transition toward a mixed model of service delivery; (3) weak accountability mechanisms and the erosion of professional norms in the health sector; and (4) the impact of HIV/AIDS. The discussions underline the need to base policies on a micro-analysis of how health workers make constrained choices, both in their career and in their day to day professional activities.
