Abstract

lnflammatory bowel diseases (Crohn's disease and ulcerative colitis) are inflammatory states occurring in the gastro-intestinal tract. The incidence of these diseases is steadily increasing in the world. Conventional therapies used nowadays have multiple side-effects. There is therefore a strong interest to develop new approaches making it possible to treat these diseases while reducing the side effects. The cannabinoid system is primarily composed of two endocannabinoids (the anandamide and the 2-AG). oftwo cannabinoid receptors (CB1 and Cfü) and of two enzymes of degradation (FAAH and MAGL). Thissystem plays a paramount role in the gastro-intestinal system. thus explaining its interest in inllammatorybowel diseases. The therapeutic use of cannabis is l imited because of its psychotropic effects. Numerous alternative acting d irectly on the cannabinoid system are under development: direct and indirect synthetic cannabinoids or natural cannabinoids extracts from Cannabis satim. All of these studies aim to treat inflammation peripherally while avoiding central side-effects and cannabis dependancy. Les maladies inflammatoires chroniques de l'intestin (maladie de Crohn et colite ulcéreuse) sont des états inflammatoires survenant dans le tractus gastro-intestinal. L'incidence de ces maladies ne cesse d'augmenter dans le monde. Les thérapies conventionnelles utilisées à ce jour possèdent de multiples effets secondaires. C'est pourquoi, il est intéressant de développer de nouvelles approches permettant de traiter ces maladies tout en diminuant ces effets indésirables. Le système cannabinoïde est essentiellement composé de deux endocannabinoïdes (l'anandamide et le 2-arachidonoylglycérol), de deux récepteurs aux cannabinoïdes (CB1 et CB2) et de deux enzymes de dégradation (FAAH et MAGL). Ce systèmejoue un rôle prépondérant au niveau du système gastro-intestinal , ce qui explique son intérêt dans les maladies inflammatoires de l 'intestin. L'utilisation du cannabis en thérapeutique est limitée à cause de ses effets psychotropes . De nombreuses approches agissant directement au niveau du système cannabinoïde sont en cours de développement : des cannabinoïdes synthétiques directs et indirects ou encore des cannabinoïdes naturels extraits du Cannabis saliva. L'ensemble de ces études visent à traiter l'inflammation de manière périphérique tout en évitant les effets secondaires centraux du cannabis ainsi que la dépendance.

Abstract

Ulcerative colitis (UC) is one of the major forms of inflammatory bowel disease (IBD). Epidemiological trends suggest an initial rise in the incidence of UC, followed by Crohn's disease (CD), in areas where IBD is emerging. As understanding of aspects of the disease is evolving, this book covers new perspectives on the etiology, clinical manifestations, diagnosis, and management of UC. Genetic predisposition, along with gut dysbiosis and environmental factors, triggers the altered gut permeability and dysregulated immune activation that leads to the development of UC, which can manifest both intestinally and extra-intestinally. Platelets play a significant role in augmenting inflammation. Histological examination is important for accurate diagnosis and for distinguishing mimics. Histological remission is an emerging treatment target strategy in UC. Novel treatments include dietary manipulation with anti-inflammatory dietary components like fermented rice bran. A number of challenges need to be addressed in treating special populations like children and pregnant women. Finally, interventional endoscopy is playing an emerging role in the management of colitis-associated neoplasia and postoperative complications and acting as a bridge between surgery and medical therapy. Ulcerative Colitis - Etiology, Diagnosis, Diet, Special Populations, and the Role of Interventional Endoscopy aims to act as a ready reference for the clinician. It provides indispensable updates on several relevant issues in the diagnosis and management of ulcerative colitis and has benefited from the collaboration of leading experts in various aspects of the disease.
