Première cause de handicap acquis chez l’adulte, l’accident vasculaire cérébral, ou AVC, peut brutalement faire disparaître ou empêcher, de façon temporaire ou non, un grand nombre de capacités de la vie quotidienne, dans des domaines physiques ou intellectuels très divers : la marche, la déglutition, la planification, la lecture, la préhension, etc. Parce qu’il touche à des savoir-faire acquis, l’AVC peut apparaître comme une atteinte biologique du social qui en efface les effets en réinitialisant les expériences, les compétences et les dispositions, autrement dit comme un accident égalisateur qui annule les différences sociales entre individus. Pourtant, à âge égal et à gravité équivalente des lésions cérébrales, les séquelles ne seront pas les mêmes si le patient est un homme ou une femme, un ouvrier ou un cadre supérieur, si la récupération de ses capacités a une grande ou une moindre valeur aux yeux des acteurs de la rééducation, si l’AVC a laissé intact chez lui un rapport aisé ou difficile aux modalités scolaires d’apprentissage. Pour mettre en évidence et expliquer ces phénomènes, Muriel Darmon a mené une enquête approfondie dans un service de neurologie d’un hôpital universitaire et auprès des différents corps de spécialistes – kinésithérapeutes, ergothérapeutes, orthophonistes, neuropsychologues, etc. – de deux centres de rééducation.
