Abstract

This is the first text to systematically review the evidence for obstetric anesthesia and analgesia. Evidence-based practice is now being embraced worldwide as a requirement for all clinicians; in the everyday use of anesthesia and analgesia for childbirth, anesthetists will find this synthesis of the best evidence an invaluable resource to inform their practice. Contributions from anesthetic specialists trained in the skills of systematic reviewing provide a comprehensive and practical guide to best practice in normal and caesarean section childbirth. Abstract

The book covers all aspects of anesthesia in newborns, neonates (under 28 days) and premature babies. These patients are highly vulnerable, very small in size and weight, at very high risk, and have high mortality and morbidity, which gets further aggravated by the medical diseases and congenital abnormalities these babies may suffer from. The book provides knowledge that equips the anesthetists with the regional techniques used to provide both anesthesia and analgesia, including information about the drugs used, skills in providing neuraxial blocks, nerve blocks, etc., complications thereof and the specially designed equipment for these patients (IV cannula, endotracheal tubes, ryles tubes, laryngoscopes, SpO2 probes, monitors, incubators, warmers and heaters, mattresses, OT tables). The book covers the pharmacology of drugs used, the effect of anesthetic drugs on the developing brain, pulmonary physiology, airway assessment and management, ventilation modes, getting IV and arterial access, preoperative workup, anesthetic management, and postoperative care. It includes care and concerns from both surgical and anesthesia aspects. It covers all the common surgical diseases along with some rare cases such as oncological concerns, palliative care, ethical concerns and pain management. The book also discusses the role and use of ultrasound. The chapter on anesthesia for short procedures includes common birth and neonatal care injuries. The book contains chapters on fluid therapy, blood and blood product transfusion guidelines and indications, resuscitation of a newborn and neonate, and neonatal rehabilitation to improve neonatal outcomes. It contains a dedicated chapter on why these babies are at a high risk of morbidity and mortality and contributing maternal factors. All the chapters are written by experts in their fields with vast experience. This book bridges the gap in the knowledge of an anesthesiologist between anesthesia for children and adults and newborns, neonates and preterm babies.

Abstract

It has been enormously satisfying to prepare the third edition of Anesthetic and Obstetric Management of High-Risk Pregnancy because it fulfills the need for close communication between two specialties, anesthesiology and obstetrics. This relationship is necessary not only to take care of the sick parturient, but also to deliver a healthy baby. New developments in obstetrics and in obstetric anesthesia necessitated this third edition. Most of the contributors to this edition are new, and they have provided information on the latest approaches, as well as updated information. In addition, Chapter 13, “Critical Care Anesthesia for High-Risk Parturients,” is completely new. Like earlier editions, the third edition includes contributions from an international team of experts in the field of obstetric anesthesia and obstetrics. I thank all the authors for their va- able contributions. The authors have expressed their own opinions and recommendations, which do not necessarily reflect my own views. I also wish to thank Ms. Elizabeth Kiernan for her endless help in completing the new edition. I sincerely hope this edition will further reinforce the concept of the team approach for taking care of the high-risk parturient.

Abstract

"This book describes in fascinating detail the history of the use of anesthesia in childbirth and in so doing offers a unique perspective on the interaction between medical science and social values. Dr. Donald Caton traces the responses of physicians and their patients to the pain of childbirth from the popularization of anesthesia to the natural childbirth movement and beyond. He finds that physicians discovered what could be done to manage pain, and patients decided what would be done."--Jacket.
