A l’heure actuelle, le cancer colorectal (CRC) représente le troisième cancer le plus commun chez l' homme et le deuxième chez la femme, et ce, à travers le monde. Rendant le développement de méthodes préventives et de traitements essentiel. J'ai étudié dans ce travail le rôle de l’aspirine sur ce type de cancer. L'aspirine exerce son rôle anticancéreux principalement via la voie des cyclo-oxygénase 2 (COX-2), en inhibant la production des prostanoïdes . La COX-2 est, en effet, souvent impliquée dans le développement des cancers. En prévention, les effets de l'aspirine sont faibles voire inexistants. Ils sont, néanmoins, plus élevés lors de la présence de certaines mutations ou modifications d'expression de gènes, mais l'aspirine ne semble pas apporter d'avantages par rapport aux autres méthodes préventives utilisées. Par contre, l'usage de l'aspirine en traitement montre des effets positifs sur la mortalité globale chez les personnes ayant un CRC. Les résultats sont cependant moins clairs pour la mortalité spécifique au CRC, si ce n'est chez les personnes exprimant la PBKCA muté ou sur-exprimant la COX-2. Ainsi, l'aspirine représenterait un traitement cout-efficace, et plus particulièrement, aux stades 1 et II du développement tumoral.En conclusion, l'aspirine semble avoir des effets positifs dans Je traitement des CRC mais des investigations supplémentaires restent indispensables avant d'envisager son usage. En outres les effets secondaires doivent être évalués par rapport aux effets positifs afin de déterminer si son utilisation présente un réel avantage dans le traitement du cancer colorectal. Colorectal cancer (CRC) is the second most common cancer in men and third most common in women. Research for new treatment and preventive therapy is essential. Aspirin inhibits the cyclo oxygenase (COX) enzymes and in particular COX-2. COX-2 inhibition prevents the production of prostanoïds, in particular PGE2, often involved in carcinogenesis. As chemoprevention amongst the general population, the benefits of aspirin are weak or non -existent. However, they are higher with the presence of some mutation s or modifications of gene expression although aspirin isn 't shown to be more effective than other preventive therapies However , in the field of adjuvant therapy, the use of aspirin has shown a definite reduction in global mortality for patients with CRC'. The result on CRC specific mortality is still unclear, with the exception of patients with Pl3KCA m mutations or with COX -2 overexpression. As a widely available medication, aspirin could be a cost-effective therapy, particularly in stage I and I l of colorectal cancer, and may have a potential role in the arsenal of adjuvant chemotherapeutic agents for CRC'. However, further studies will be needed to show if its benefit is truly significant. Moreover, the side effects need to be considered in balance with the potential benefits of aspirin in CRC'.
