Opioids are a family of analgesics well known for their strong efficiency in relieving pain. Nevertheless, they present a list of side effects that can endanger the patient’s life. What alternatives to the use of opioids are the therefore available? How is it possible to counter the side effects while relieving pain in the best way? In order to answer this question a recent concept based on the substitution of an opioid by another has emerged: the opioid rotation. This technique prevents in an efficient way the apparition of side effects while relieving the pain optimally. It is suitable during long term treatments and therefore opioid rotation is relevant in cases of chronic pain met for example in oncology and in palliative care. This master thesis aims to enlarge the reader’s knowledge on the opioid rotation concept, through theoretical notions and illustrated in practice by clinical studies. Les opiacés sont une famille d'analgésiques connus pour leur grande efficacité dans le soulagement de la douleur. Néanmoins, ils présentent une série d'effets indésirables pouvant conduire à un réel danger pour le patient. Dès lors, quelles alternatives sont envisageables à l'utilisation des opiacés ? Comment contrer les effets négatifs des opiacés tout en soulageant au mieux la douleur du patient ? Face à cette question, un concept récent basé sur la substitution d'un opiacé par un autre a vu le jour : la rotation des opiacés. Cette technique permet non seulement de contrer au mieux l'apparition des effets indésirables mais aussi de soulager de manière efficace les douleurs du patient. Adaptée lors de traitements sur le long terme, la rotation des opiacés prend tout son sens dans le cas de douleurs chroniques, rencontrées par exemple au sein des services d'oncologie et de soins palliatifs.Ce mémoire a pour but d'élargir la connaissance du lecteur sur le concept émergent dE la rotation des opiacés au travers de notions théoriques et pratiques par la mise en avant d'études cliniques.
